1.Inleiding
- Normale degeneratieve veranderingen – grijze zone – lumbale pathologie
- Zenuwstructuren:
o RM (niveau L1-L2)
o Conus medullaris (achter wervels L1-L2)
o Cauda equina (vanaf L2-L3)
Bepaalt de symptomatologie:
Letsels RM: UMNS
Letsels cauda equina: wortelsymptomen
- RM: dorsale en ventrale wortels  komen samen in het foramen intervertebrale  vormen
de spinale zenuwen:
o Ventrale ramus  lumbale en lumbosacrale plexus
o Dorsale ramus  3 takken / bezenuwt posterieure helft van het wervellichaam,
paraspinale spieren, facetgewrichten (mediale tak) en lumbale multifidi
- LWZ: wortel komt ONDER de wervel  tssn L4-L5 komt wortel L4 naar buiten komen
- Myotoom =
o Alle spieren die geïnnerveerd worden door de ventrale wortel van één en dezelfde
radix
o De meeste spieren worden door meerdere radices (in verschillende mate)
bezenuwd: behoren dus tot verschillende myotomen (overlapping)
- Dermatoom =
o Huidgebied dat door één radix bezenuwd wordt
o Belangrijke overlapping tussen opeenvolgende dermatomen
- Innervatie  pijngevoelig
o Anulus: buitenste 1/3
o Longitudinale ligamenten
o Facet (posterieure kapsel; synoviale plooien; subchondraal bot)
o Periost
o Dura mater: anterieur
- Niet-geïnnerveerd
o Nucleus pulposus
o Articulair kraakbeen
o Ligamentum flavum

2.Lumbale pathologie
2.1. Anatomische varianten/afwijkingen
2.1.1. Spina bifida
- Neurale buis defect = malformatie van 1 of meer wervels
- 3 vormen:
o Spina bifida occulta
o Meningocele

, o Myelomeningocele = RM zak puilt uit
 Associatie met hydrocephalie
 Verlamming OL
 Klompvoet
 Incontinentie
 Tethered cord
- Relatie rugpijn? Scoliose? Bedwateren?
- Hypertrichosis = uitwendig harig plekje thv. lage rug
- Hyperpigmentatie = donkere vlekjes op de huid

2.1.2. Nauw spinaal kanaal
- Puber, niveau L5

2.1.3. Asymmetrische facetgewrichten
2.1.4. Sacralisatie van L5-lumbalisatie van S1-neoarticulaties
- Soms maar 4 lumbale wervels  5e lumbale wervel zit in sacrum  sacralisatie van L5
- Soms 6 lumbale wervels  1e sacrale wervel komt uit sacrum  lumbalisatie van S1
- Onderzoek: RX lumbaal
- Gevolg? Niveaus kloppen niet  referenties die je wel kan gebruiken:
o Rib 12
o Pedikel L3 (processus transversus steekt meest lateraal uit)
Zo kan je zien of het een lumbalisatie of een sacralisatie is
- Neoarticulatie = 1 kant processus transversus L5 zit vast aan sacrum
Kant die los zit geeft het meeste problemen

2.1.5. Spondylolyse
- Onderbreking van de wervelboog (pars intra-articularis)
- Types:
o Congenitaal (dysplastisch)
o >>> isthmische spondylolysis
o Degeneratieve spondylolysis
o Traumatische
o Post-chirurgische
o Pathologische
- Diagnose:
o RX-3/4e opname  Maakt geen onderscheid tussen
isthmische/congenitale, daarvoor moet je petscan doen
o MRI (vroeger botscan)
- Conservatieve therapie
o Enkel bij klachten  indien asymptomatisch: geen restricties
o Relatieve rust  3 maand (sport niet meer uitoefenen)
o Informeren!
