How did the simulated experience of Randy Adams’ case make you feel?

I felt a little uncomfortable with Randy’s case. First of all, I felt genuinely bad for him. You can tell
that he is struggling to cope with what is happening to him. As someone who has suffered from
concussions, I know that alone can be overwhelming. When you add PTSD to the mix, I can see
how that would be unbearable. Secondly, he verbalized early on that he did not want to be
asked a lot of questions. However, to get the needed information, I had to continue to ask him
questions. I know that we have to complete assessments to better assist the patient, it just felt
weird to continue when he was stating he was already overwhelmed.

Talk about what went well in the scenario.

I felt like I was able to move through the encounter with more ease than previous encounters. I
knew what needed to be asked to complete what the provider was looking for and what the
patient needed. Moving through the encounter felt more natural and not just like checking off
the boxes, it felt like a real conversation where I was gathering information as we went.

Reflecting on Randy Adams’ case, were there any actions you would do differently? If so, what were
these actions and why?

I maybe would have asked his wife to rate how different he had been as well. When asked he
stated that he was maybe a 1, with 0 being normal ad 6 being different. However, he mentioned
that Joy is the one who noticed him being different. I’m curiuous if she would also rate him a 1.
He mentioned also that he has difficulty remembering things and that he relies on Joy a lot, so
maybe asking her if there was anything else she had noticed about his behavior that he was not
reporting.

PCC/S What could occur if Randy Adams kept taking his sumatriptan succinate?

Continuing his sumatriptan succinate longer than the recommended period of time can actually
lead to rebound headaches that may get worse or even occur more frequently. Also, since it is a
serotonin agonist, there is also a risk of serotonin syndrome if the levels get too high in the body.
Also there could be an increase in sensitivity to noise and light.

PCC/I Discuss the types of support groups and resources that would be beneficial to Randy Adams and
his family.
