Kunnen ingezet worden bij preventie of behandeling van:
o Maligne osteolytische aandoeningen (ziekte van Kahler)
o Botmetastasen (mammacarcinomen, prostaatcarcinomen)
Behandeling leidt tot afname (17%) van kans op ontwikkelen van skelet
gerelateerde complicaties.
o Longtumoren
o Bij behandeling van postmenopauzale osteoporose.
Vooral oraal: alendroninezuur, risedroninezuur en ibandroninezuur.
Leidt tot afname van aantal nieuwe wervelfracturen (40-70%).
Werking: kunnen de vicieuze cirkel van osteoclastenactivatie, botafbraak en tumorstimulatie
doorbreken.
Probleem toepassing
o Verhoogde kans op ontwikkelen van osteonecrose (fig. 23.1)
o Leidt tot destructie van uitgebreide kaakdelen.
o Gevolg: verlies gebitselement of lokale gebitspathologie (parodontaal verval,
periapicaal granuloom).
o Wanneer intraveneuze toediening van bisfosfonaten is geïndiceerd, moet er een
tandheelkundig focusonderzoek verricht worden en alle foci voor aanvang van de
therapie verwijderd worden.
o Risico op ontwikkelen van osteonecrose bij oraal gebruik is geringer dan bij
intraveneuze toediening (tabel 23.1)
o Staken van bisfosfonaten voorafgaand aan thk behandeling weinig zin, effect
van bisfosfonaten gaat jaren door na staken van therapie en risico op ontwikkelen
van osteonecrose blijft.
