Wat is diabetes type 1:
- De betacellen produceren geen insuline meer, patient is dus afhankelijk van insuline
spuiten
- Destructie beta cellen door een autoimmuunproces
- Omgeving factoren van invloed: virus, chemicalieen, voedingsmiddelen en mogelijk een
tekort aan vitamine D
- De spieren, lever en vetweefsel zijn gevoelig voor insuline. Bronnen van glucose zijn
voedsel en de lever (gluconeogenese is niet zo gunstig er worden dan eiwitten
afgebroken, en glycolyse)
Er zijn enkele aandoeningen die vaker voorkomen bij DM1; auto-immuun geinduceerde
hypothyreoidie (25%), polyendocriene syndromen en coeliakie
Insuline therapie:
- Standaard bij type 1, soms ook bij type 2 en zwangerschapsdiabetes
- Het doel is om de glucosespiegel binnen normaal fysiologische grenzen te houden ter
voorkoming van chronische complicaties (diabetes control and complications trail)
- Insuline wordt subcutaan toegediend, instelling gebeurd in het ziekenhuis door
internist in samenwerking met diabetesverpleegkundige
- Normaal geeft een betacel een variabele hoeveelheid insuline af, afhankelijk van je
maaltijden. Insuline moet die variatie nabootsen van buitenaf.
- sinds 1922 insuline uit de alvleesklieren van varkens, sinds 1982 door bacterien
(humane insuline)
- Humane Insuline piek 3 uur tot 6-8 uur werkin, NPH insuline: piek 4-6 uur duur 12-16,
Insuline analogen: ultrakort 1996-1999 piek 2 uur duur 4-5 uur, Detemir en glargine:
duur 24-48 uur
