Introduction
Een belangrijk probleem waarmee klinische neuropsychologen te maken krijgen, is het
krijgen van financieel vergoeding voor de diensten die zij leveren. De ‘poortwachters’ van
financiële vergoeding hebben vaak professionele en economische redenen waarom ze wel of
niet betalen voor neuropsychologische diensten. Om klinische neuropsychologie als een
beroep te laten floreren, zijn twee soorten ‘data’ of bewijs’ nodig. Eén type data is afkomstig
van goed ontworpen, ‘evidence-based’ uitkomststudies die de effectiviteit en de
kosteneffectiviteit van onze procedures en interventies aantonen. Een ander cruciaal type
‘data’ dat nodig is, is dat individuele praktiserende/beoefende clinici de verwijzende artsen
duidelijk de waarde van hun klinische oordeel in de patiëntenzorg laten zien.
Neuropsychologen beschikken over unieke klinische en professionele vaardigheden die
zinvolle informatie kunnen toevoegen aan casusconceptualisatie en invloed kunnen hebben
op de patiëntenzorg, wat een echt verschil kan maken voor artsen. Deze vaardigheden en
het unieke vermogen van ons beroep om informatie toe te voegen aan
casusconceptualisatie moeten de bewoners duidelijk geleerd worden via een mentormodel,
zodat zij deze informatie kunnen begrijpen en communiceren met de leveranciers.
Als we de noodzaak van deze twee verschillende bronnen van ‘bewijsmateriaal’ niet serieus
nemen, verliest ons beroep economische voet aan de grond in een toch al worstelende
gezondheidszorgeconomie. Het eindresultaat is tweevoudig. Ten eerste zullen cliënten of
patiënten alleen gezien worden op basis van een ‘vergoeding voor service’, die slechts
enkele families kunnen betalen. Het resultaat zou zijn dat sommige patiënten die
neuropsychologische diensten nodig hebben, deze niet zouden krijgen, wat zou kunnen
leiden tot uitstel van de juiste diagnose, gebrek aan juiste behandelingsinterventies of
andere negatieve resultaten die vermeden hadden kunnen worden als neuropsychologische
beoordeling deel uitmaakte van de klinische en diagnostische opwerking. De andere impact
zal een groeiende trend zijn van neuropsychologen om meer ‘medisch-legaal’ werk te doen
om economisch te overleven. Hoewel dit op zichzelf niet negatief is, begon
neuropsychologie als een klinische wetenschap en om ons beroep te laten groeien, moeten
klinische neuropsychologen actief blijven in het bieden van zowel patiëntenzorg als
onderzoek dat relevant is voor ons vakgebied. Het is om deze reden dat we serieus moeten
evalueren wat we doen, hoe goed we erin zijn en wat de waarde van ons werk is.

Professional identity of neuropsychologists
Harold Goodglass (1994) merkte op dat ‘klinische toepassing van neuropsychologie even
dicht bij of dichter bij zijn wetenschappelijke wortels ligt dan enig ander specialisme binnen
de psychologie’. Neuropsychologen bloeiden omdat ze hun discipline hebben kunnen
relateren aan een groeiende database in de neurowetenschappen. Deze technische kennis is
onmisbaar voor ons werk, maar het is een onvoldoende voorwaarde voor een succesvolle
praktijk van klinische neuropsychologie. We moeten ook goed zijn in het nemen van deze
