Les 1: Fysische revalidatie en kiné

Definitie

Revalidatie = het geheel van alle medische, paramedische en sociale hulpmiddelen die toelaten aan
een patiënt met een bestaande of dreigende handicap

(1) om de functionele, psychologische en socioprofessionele gevolgen van deze handicap zo veel
mogelijk te beperken en

(2) een zo autonoom mogelijk bestaan te lijden

Transversaal, multidisciplinair, coordinatie

ICF: internationaal classification of functioning – WHO 2001 (heel goed kennen + kunnen tekenen)

Met menselijk functioneren omschreven vanuit drie domeinen:

Het menselijke organisme = anatomische
structuur en orgaanfucntie

Probleem = stoornis (impaiment)

Het menselijk handelen = acutiviteiten

Probleem = beperking (limitation)

Mens als deelnemer van maatschappeijk
leven = participatie

Probleem = participatieprobleem (restriction)

Alle 3 beïnvloed door context: omgevevings- en persoonlijke factoren

,Grondslag van de revalidatie en laat toe objectieve functionele doelstellingen te identificeren.

Rehab cycle

Toelatingsvoorwaarden voor multidisciplinaire revalidatie

• Lijst van pathologieën waarvoor pluridisciplinaire revalidatie wordt terugbetaald
• Cognitieve functie voldoende om nieuwe zaken te leren en instructies te begrijpen
• Communicatie mogelijkheden voldoende om samenwerking met de zorgverleners
mogelijk te maken
• Fysische conditie voldoende om een actief revalidatieprogramma( tot2u/dag) aan te
kunnen
• Realiseerbare therapeutische doelstellingen
• Andere factoren te beschouwen<> revalidatie
 Functionele toestand VOOR letsel?
 Sociale situatie
 Gemeenschappelijke doelstellingen patient EN zijn omgeving (familie,…)

Voorschrift kinesitherapie (heel goed kennen)

1. Naam, voornaam, geboortedatum

2. Diagnose

– Vb. tendinopathie, ...

3. Localisatie

, –Vb. rechterschouder, ...

4. Aantal zittingen
– Maximum 18 zittingen op eenzelfde voorschrift
– Maximum 18 zittingen per jaar voor eenzelfde (lichte) pathologie
– Meestal 9 zittingen en daarna herevaluatie door de arts
– Beschouwd als maximaal aantal (kiné mag minder, maar niet meer zittingen uitvoeren)

5. Frequentie
– 1 tot meerdere keren per week
– Respiratoire pathologie bij kinderen: 2x/dag
– Beschouwd als de maximale frequentie (kiné mag minder, maar niet meer zittingen
uitvoeren)

6. Type behandeling
– Massage
– Mobilisatie
– Oefeningen: rekoefeningen, versterkende oefeningen (isometrisch, excentrisch,...),
evenwicht, proprioceptie, cardio-training, stappen/transfers, posture correction,...
– Thermotherapie, cryotherapie, electrotherapie

7. Contra-indicaties (facultatief)

De kinesitherapeut kan zelf bepalen of een korte of een langere zitting nodig is ifv het type
pathologie: tarief ifv duur vb. M24 = 30 min behandeling; M16 = 20 min behandeling; M12 = 15
min behandeling

((( Licht pathologie: max 18 per jaar

Functionele pathologie: max 60 (lijst acuut is Fa en lijst chornische is Fb)

Zwarte pathologie: max 120 ( E lijst). )))

Kine voor functionele pathologie (F)

• RIZIV-nomenclatuur F-pathologie
• Mits akkoord adviserend geneesheer op basis van:
o Diagnose bevestigd door geneesheer-specialist
o Aangetoond verband tussen diagnose en functionele handicap
o Nauwkeurige evaluatie en behandelingsplan door kinesitherapeut
• Akkoord voor max. 1 jaar
• Maximaal 60 zittingen per jaar (behalve polytrauma: 120)
• Twee lijsten:
o Acuut (intense kinesitherapiebehandeling gedurende een korte periode)
o Chronisch (regelmatige kinesitherapie behandeling die meerdere jaren kan
duren)
