PROJECT DESCRIPTION

, The HIP was to developed to improve patient safety and outcomes by enhancing

communication during the pre-case planning between the Cardiothoracic Surgeons,

Interventional Cardiologists, and the medical device valve specialist prior to the

Transcatheter Aortic Valve Replacement (TAVR) procedure. The issue identified during the

analyses of the TAVR pre-case planning was variances in communication and exchange of

information. At times, pertinent patient information was excluded, which can lead to

patient safety concerns. The opportunity was to create a standardized pre-case planning

form that will be utilized prior to every TAVR procedure. Utilizing the pre-case planning

form will standardize and enhance team communication, decrease missed patient

information prior to the procedure, validate aortic valve size, and ensure necessary

equipment is available for the procedure. Key stakeholders from the Structural Heart Team

and the medical device company will come together and discuss what information should

be included on the pre-case planning form. Once the form is finalized, education will be

given to the staff involved, and data will be collected to determine the success of the

project. The HIP addresses the identified problem by creating a standardized pre-case

planning form that the TAVR team will utilize before each TAVR procedure.

LIFECYCLE PHASES MAJOR TASKS AND ACCOMPLISHMENTS
Initiation Phase A needs assessment was completed using the fishbone diagram during
the initiation phase. The needs assessment exposed the lack of standard
pre-case planning and a communication gap between the teams. Key
stakeholders were identified, including SMEs for TAVR procedures. Once
the team was established, a brainstorming session was held, and the
team identified that a HIP creating a standardized pre-case planning
form would benefit the organization and patient care. The team
developed the SMART goal for the HIP, performed a SWOT analysis, and
impact analysis to see if the HIP benefited the organization. Once these
analyses confirmed that the HIP was beneficial, the team engaged in
brainstorming sessions.
