1 ECMO
1.1 Inleiding

1.2 Wat is ECMO?”

1.3 Soorten ECMO?
VV-ECMO

Reeds beademde patiënt met ernstige hypercapnische of hypoxische problemen
Respiratoire ECMO (~ kunstlong)

Alle organen worden van dezelfde zuurstofbevoorrading voorzien
Duidelijk kleurverschil in het bloed te zien (donkerrood voor membraanoxygenatie)
Indicatie:
1) ARDS (acute respiratory distress syndrome) = falende longen
P/F ratio bepalen adhv bloedgasanalyse = PaO2 en Fio2
o PaO2: 75-100mmHg
o Fi02: 21% in de omgeving (0,21); vb. 90% bij een beademde pt (0,9)
Waarden analyseren
o P/F ratio normaal: 452
o Milde ARDS: 200-300
o Matige ARDS: <200
o Ernstige ARDS: <100
2) Hypoxische respiratoire insufficiëntie = uitgesproken zuurstoftekort
Voornamelijk veroorzaakt door een ventilatie/perfusie mismatch (V/Q)
o Normaal 1/1
o Slechte perfusie <1
Plaatsingsmethodes:

- Drainage: v. femoralis Plaatsing via vena jugularis

Ondersteuning hart OF hart & longen
Onafhankelijk vd pompfunctie vh hart
Bloed ontnemen via veneuze weg
O2 rijk bloed terug in het arteriële systeem
+++ kans complicaties// door arteriële toegang
Indicatie:
1) Lage CO en hypotensie met sys RR <90 mmHg, ondanks gebruik hoge dosis inotropie
Bij cardiogene shock met myocarddisfunctie
CO of HMW = slagvolume x frequentie
Cardiomyopathie tgv sepsis
+/- 4-8L/min
Pulmonale hypertensie met RV-falen
2) In afwachting van harttransplantatie
Aandachtspunten:
Vooral bij een hart dat nog weinig doet
Voorwaarde: aortaklep moet nog openen en sluiten – coronairen moeten zich kunnen vullen
o Nog een zekere golving op arteriële drukcurve// aortaklep ok
o Sowieso wel al een vlakker verloop dan normaal
Onvoldoende opengaande aortaklep =
o Instellen lagere bloedflow, opdrijven inotropie, gebruik IABP, plaatsen Impella
