Risico-inschatting Er is nog geen probleem maar we denken afhankelijk van de aandoening die
iemand heeft dat het een risico is.
PR-structuur: probleem en risico.
P: probleem. Het probleem is altijd de verpleegkundige diagnose.
R: risicofactoren. Mogelijke factoren die ervoor kunnen zorgen dat een
zorgvrager de verpleegkundige diagnose kan krijgen.
- Pathofysiologisch: auto-immuunziekten, allergieën, neurologische schade
met verlies van sensorische of motorische reflexen, oppervlakkige schade
aan het oog, vitamine A-deficiëntie, omega-3-vetzurendeficiëntie,
medicatie zoals ACEremmers, antihistaminica, diuretica, steroïden,
antidepressiva, tranquilizers, analgetica, sedatieven, neuromusculaire
blokkers, chirurgische ingrepen, beademing, airconditioning,
luchtverontreiniging, cafeïnegebruik, minder knipperen, onvoldoende
kennis over beïnvloedbare factoren, onjuist gebruik van contactlenzen,
onjuist gebruik van ventilator, onjuist gebruik van haardroger,
onvoldoende aandacht voor meeroken, onvoldoende vochtinname, lage
luchtvochtigheid, roken, blootstelling aan zonlicht, gebruik van producten
met conserveermiddel benzalkoniumchloride, omgevingsfactoren, heet,
droog en winderig klimaat, woonplaats, vrouwelijk geslacht, levensstijl en
ouderdom.
- Dat er geen droog oog ontstaat of dat het droge oog minder droog wordt.
- Dingen die een zorgvrager kan doen om een droog oog te voorkomen.
- NIC zijn alle verpleegkundige interventies en NOC is wat je wil bereiken.

R: mevrouw is allergisch voor katten, binnenkomt komt er een kat bij haar in
huis.
