Casus
Meneer de Lier van 49 jaar ligt opgenomen in het ziekenhuis. Meneer had een acute pancreatitis ten
gevolge van een post-hepatische (cholestatische) icterus bij chronisch alcoholmisbruik. Meneer heeft een
behandeling ondergaan (ERCP), waarmee de galstenen zijn verwijderd. De obstructie van de galwegen
is hiermee verholpen en meneer begint hier langzaam van op te knappen.
Echter blijven bepaalde bloeduitslagen afwijkend. De verpleegkundige vermeldt tijdens de visite dat
meneer de Lier 1,5 kg is aangekomen sinds de opname waarbij vooral de buikomvang lijkt toe te
nemen. Daarbij had meneer een bloeddruk (tensie) van 180/95. Bovendien merkte zij vanochtend tijdens
het verzorgen op, dat meneer de Lier wat moeite had om zijn tanden te poetsen en aan te kleden maar
ook het communiceren gaat lastiger dan enkele dagen eerder. De arts besluit op basis van deze
bevindingen bloed te prikken om de leverfunctie te bepalen bij verdenking beginnende levercirrose.
Uit het bloedonderzoek blijkt dat de stolling van het bloed niet goed is, dat het billirubine en ammoniak
verhoogd is. Het albumine is verlaagd.
