De lage rug heeft vijf wervels (L1-L5). Dit zijn de grootste wervels van de hele wervelkolom.
Deze vijf wervels dragen het gewicht van het bovenlichaam. Bovendien is het ook een heel
beweeglijk deel van de wervelkolom. Het hoge gewicht in combinatie met de grote
beweeglijkheid, zorgt ervoor dat deze wervels het snelst beschadigd raken met als gevolg
lage rugpijn.

Lage rugpijn wordt doorgaans ingedeeld in specifieke en aspecifieke lage rugpijn.

- het lumbosacraal radiculair syndroom: een vorm van specifieke lage rugpijn met radiculaire
pijn in 1 been, die al dan niet gepaard gaat met neurologische uitvalsverschijnselen.
- rugpijn als gevolg van een mogelijk ernstige onderliggende specifieke aandoening, zoals
(osteoporotische) wervelfracturen, maligniteiten, spondylitis ankylopoetica, ernstige vormen
van kanaalstenose, of ernstige vormen van spondylolisthesis.

Radiculair syndroom
Dit is een verzamelnaam voor de pijn die ontstaat door compressie van een zenuwwortel
(radix) in de rug. De zenuwen kunnen hierdoor minder goed signalen doorgeven, zo worden
spieren minder goed aangestuurd (krachtsverlies) en ook het gevoel wordt verstoord
(prikkelingen of doof gevoel). Aangezien lumbale en sacrale wervels naar de benen toe
lopen, komen hier de symptomen tot stand. De meest voorkomende oorzaak van een
lumbosacrale radiculair syndroom is de hernia nuclei pulposi (HNP). Een ander oorzaak is de
lumbale spinale stenose (vernauwing in de lendenwervels).

Mogelijke risicofactoren lage rugklachten:
- Zwaar lichamelijk werk (vooral indien dit gecombineerd wordt met frequent vooroverbuigen
en draaien van de wervelkolom)
- Zwaar lichamelijk werk
- Mentale stress
- Langdurig roken
- Frequent langdurig autorijden (>2 uur per dag, meer dagen per week)
- Genetica

Klachten en symptomen radiculair syndroom
- Pijn in de rug met uitstralende pijn/doofheid/tintelingen naar extremiteiten
- Uitstralende pijn tot voorbij de knieholte
- Zeer heftige pijn in been, vaak met een scheut
- Vooral pijn bij lang zitten, vooroverbuigen en hoesten/niezen/persen
