Define the concept of Person-centered practice.
Examples must be used to support the definition.
Being a health and social care worker, we respond to
all the needs our client have and make the best care
provided. One way in doing so is through the person-
centered practice. A person-centered practice is an
approach to help your service user by including them
through all the planning, decision making and services
they may need in order to have the best possible
outcome in their situation. Communicating with them
and asking their opinions, needs, hopes and what they
want may contribute to this practice. They must know
that their opinions or ideas are recognised in the plan
of care that is made, to be able to maximize the
person-centered practice and make the delivery of
care as effective as possible. But, you must clarify to
them that this practice is a two way approach, meaning not all of what they want will happen.
It is our responsibility to let them understand that, by giving them health education about
their ideas so they will not take it badly and be more informed about their condition and
make better decision and choices.
An example of this is with a service user who is a drug addict that wants to stop or
rehabilitate. In order to help them properly, we must include them in making the plan of care
so they will understand that their ideas, needs and wants are recognised. We must also
identify if they did something before in order stop using drugs so we won’t repeat any
mistakes and as their health care worker, we must also voice out our opinions and
approaches to them so they will know what will happen or to know if they want to participate
in it or not so changes can be made that is suited to their best interest. We should also teach
them on how to stop and the effects of stopping immediately so they will be aware of any
changes and understand the process.
But, not all of the service users are able to express themselves properly. An example are
those who have visual and hearing impairment. In this case, an Advocate may be needed
through the use of advocacy services. An Advocate may be your family member, someone
who is a professional or a friend who will speak on behalf of you to promote your rights,
provide you with the best choices you need and ensure your wants and needs are heard or
given.
Doing this kind of practice is not easy for we will encounter
different service users with different conditions, wants and needs.
