Lymfoedeem = Een chronische aandoening die onderhouden moet worden door behandeling
Bovenste lidmaat lymfedrainage:
- Vnl. bij borstkanker
→ Lymfeknopen worden weggehaald bij de
operatie waardoor er schade is aan het
lymfestelsel
- 1/5 borstkanker patiënten ontwikkelt
lymfoedeem
Grootste lymfeknopen bovenste lidmaat:
- Oksel (Axillaris)
- Hals (Cervicalis)
- Elleboogplooi (cubitalis*)
- Binnenkant humerus
*Ligt 3cm proximaal van epicondylus medialis
Grootste lymfeknopen onderste lidmaat:
- Inguinale
- Knieholte (popliteale)
→ Vocht wordt langst de suprapubische regio naar de andere zijde gebracht
Grote vaten lymfe = Collectors
Lymfoedeem = Vocht in het interstitium
Stardust = Vocht dat in het interstitium lekt Huid

Normaal transport:
- Langs de collectoren
- Van distaal naar proximaal
→ Omgekeerd = Dermal Backflow
→ Vocht kan nog meer zakken door de zwaartekracht
Borstkanker patiënten:
- Schade aan lymfeknopen in de oksel (door operatie)
- Vochtophoping
→ Doel: Vocht laten omleiden naar de retroclaviculaire lymfeknopen via de Baan van
Mascagni of naar de andere oksel via de ventrale / dorsale anastomose
, Baan van Mascagni: Van de deltopectorale groeve naar retroclaviculair
4 Handgrepen van lymfedrainage
Doel = Vocht van de aangedane lymfeknoop verplaatsen en vullen in functionele lymfeknopen
- Pinch-Test:
- Altijd uitvoeren! (ook als op examen staat waar lymfoedeem zich bevindt)
- Bilateraal vergelijken
- Localiseren van het lymfoedeem
- + als huid niet ophefbaar is
- Leegmaken v/d lymfeknopen:
- Circulair (= aangenamer) met 2 vingers
- Flush:
- Collector- of Interstitiële flush (vocht naar proximaal duwen maar blijft in het
interstitium)
- Collector flush: zachte strijkbeweging over de huid (hele omtrek!)
- Fill-In:
- Vocht van het interstitium in het lymfestelsel krijgen
- = Rol + huidverschuiving met beide duimen
- Brengt rek op de ankerfilamenten die aan de huid hangen en zorgt dat de openingen
in de lymfen groter worden. Dit leidt tot beter transport (van collageen en proteïnen)

Techniek
Voorbereiding: P. goed leggen (hier staan al punten op)
→ Kussen onder de knieën, comfort
Elke handgreep 5x herhalen (maar hoe meer hoe beter)
CASUS (Vocht van de knie tot de enkel)
1. Pinch Test
2. Leegmaken inguinale lymfeknopen: langst beide kanten (5x)
3. Collector Flush naar andere zijde in zijlig (gezwollen zijde boven): ventraal (SIAS – SIAS) en
dorsaal (SIAS – Lumbaal)
4. Collector Flush van het bovenbeen: van knie tot heup (ruglig met knieflexie)
5. Leegmaken inguinale lymfeknopen: want vocht is naar boven gestuurd
6. Leegmaken popliteale lymfeknopen (knieflexie)
Onderbeen is gezwollen en dit wordt opgedeeld in 3 delen: bovenste deel, onderste deel en
voet/tenen
7. Interstitiële flush bovenste deel van het onderbeen (tafel omhoog!)
8. 5x Fill-In
9. 5x Collector Flush 2x!
10. Leegmaken popliteale lymfeknopen
11. Collector Flush naar inguinale
12. Leegmaken inguinale lymfeknopen
13. Collector Flush naar andere zijde
