, H1: INDIRECTE RESTAURATIEVE PROCEDURES


INLEIDING


• Examen:
− Het examen van het OLA van K. Van Landuyt en K. Vandamme (5sp) is deels schriftelijk en deels mondeling. Punt op 20, waarvan:
➢ Reproduceer- en beredeneervragen (n=3). Schriftelijk. Punt op 12.
➢ Opmaken van het behandelingsplan van een klinische casus. Schriftelijk, met mondelinge toelichting. Punt op 4.
➢ Herkennen en interpreteren van beeldmateriaal (n=10), gezien in het hoorcollege. Schriftelijk. De docent stelt bijvragen. Punt op 4.
− Het examen van het OLA M. Peumans en B. Van Meerbeek (1sp) is schriftelijk en omvat 1 vraag. Punt op 20.
− De finale score voor dit OPO (bestaande uit de 2 OLA's) wordt bepaald als het gewogen gemiddelde behaald op beide OLA's.


• Restauratieve tandheelkunde:
− Combinatie van conserverende tandheelkunde en prothetische tandheelkunde.
− Spectrum van restauratieve tandheelkunde:
a) Bleken (vitaal en avitaal) van verkleurde tanden.
b) Minimaal invasief en esthetisch herstel van tanden aangetast door cariës of trauma dmv directe adhesieve restauraties.
c) Uitgebreide tandrehabilitatie dmv directe, semi-directe en indirecte adhesieve restauraties (correctie tandstand, kleur en vorm).
a) Rehabilitatie extreem afgesleten dentitie dmv directe, semi-directe en indirecte adhesieve restauraties met beetverhoging.
b) Restauratie van avitale tanden en elementen met weinig restweefsel dmv indirecte restauraties (tandgedragen kronen en bruggen)
c) Vervanging van één of meerdere ontbrekende tanden dmv uitneembare prothesen
d) Vervanging van één of meerdere ontbrekende tanden dmv osseogeïntegreerde implantaten en prothetische suprastructuren
e) Volledige rehabilitatie van edentate kaken dmv osseogeïntegreerde implantaten met vaste of uitneembare suprastructuren.


− Herstel van het gebit bij:
➢ Tandweefselverlies  vaste restauraties.
➢ Tandverlies  vaste restauraties en/of uitneembare prothesen.


− Restauratieve behandelingen moeten gebaseerd zijn op:
➢ Wens van de patiënt
o Probleem esthetiek.
o Verstoorde functie (te weinig kauwcomfort).
➢ Voorkomen van toekomstige problemen of schade.
o Bv. progressieve tandschade voorkomen, beperken risico fractuur.

• Oorzaken: cariës, trauma, slijtage.
• Behandeling:

Directe − Materiaal: composiet, glasionomeer, (amalgaam).
restauratie − Voordeel: minimaal invasief, goedkoop want direct, snel.
− Nadeel: duurzaamheid vooral wanneer weinig tandweefsel overblijft, esthetiek niet altijd ideaal.

Partiële kronen − Materiaal: porselein, metaal of composiet.
− Voordeel: mooiere anatomie, knobbeloverkapping, porselein (esthetischer en duurzamer dan composiet).
− Nadeel: duur (tandtechnicus), meer weefselverlies door niet-retentieve caviteiten, meerdere zittijden nodig.
− Soorten:

− Indirecte intracoronale restauratie − Overkapt de knobbels.

en laterale bewegingen worden geleid endodontisch behandelde tanden,
door eigen cuspiden van tand. verzwakte tanden.
− Zelfde indicatiegebied als vullingen;
bieden weinig meerwaarde tov
vulling; extra weefselverlies omwille
van depouille niet-retentieve caviteit.




Solitaire kroon − Materiaal: metaal, metaal + opbakporselein (basis), keramische basis + porselein, volledig keramisch (monolithisch).
− Voordeel: volledig omvattend ⇒ duurzaamheid, esthetisch.
− Nadeel: duur (tijd, tandtechnicus, geen terugbetaling), veel weefselverlies, meerdere
zittijden nodig, esthetiek afhankelijk van gebruikte basismateriaal.

, TANDVERLIES


• Edentaten:
− Na extractie ⇒ verdere kaakresorptie (involutie) want bot volgt de tand.
− Niet voor iedereen aanvaardbaar.
− Zeer grote variatie per land: tussen 15-80% bevolking EU.
− Totaal aantal edentate patiënten neemt geleidelijk af. Ouderen patiënten behouden langer hun tanden. Het aantal ouderen neemt wel
sterk toe, dus het absoluut aantal ouderen zal nog niet afnemen.


• Behandelopties:
− Volledig edentaat:
➢ Uitneembaar: volledige prothese.
➢ Semi-vast: overkappingsprothese op implantaten.
➢ Vast: volledige vaste brug op implantaten.
− Partieel edentaat:
➢ Uitneembaar: partiële prothese.
➢ Semi-vast: overkappingsprothese op tanden.
➢ Vast: brug op tanden, solitair implantaat of brug op implantaten.
