Grote GI-bloedingen
- Pathofysiologie
o Symptomen GI-bloeding braken, bloed of bloedverlies rectaal
o Melaena (donkere ontlasting) vooral bij bovenste GI-bloeding
o Hematemesis (bloed opbraken) kan helder of donkerrood zijn. Donker bloed is
meestal al gedeeltelijk verteerd. Komt vooral door bloedingen van de oesophagus,
maag of duodenum. Komt voor bij snelle bloedingen.
- Behandeling
o Initieel elke patiënt heeft risico op een hypovolemische shock. Hoge risicopatiënten
hebben 50% kans op opnieuw bloeden tijdens ziekenhuisverblijf.
2/3e van de patiënten zijn over de 60 en 1/3e van deze patiënten gebruiken
aspirine of NSAIDs.
o Lichamelijk onderzoek, bloedverlies schatten heeft geen zin, goede veneuze toegang
maken voor bloedonderzoek. ECG en röntgen handig bij patiënten over de 65 jaar
met cardiorespiratoire aandoeningen.
- Groepering
o Hoog risicogroep monitoren, resuscitatie
(bloed/vloeistof toediening). 72u
intraveneuze PPI.
o Laag risicogroep conservatief en zullen
uiteindelijk stoppen met bloeden.
- Endoscopie
o Patiënten met bovenste GI-bloedingen
behoeven een endoscopie om de plek van
het bloeden te bevestigen. Bij hoog
risicogroep moet er binnen 12u na het
bloeden een endoscopie verricht worden.
o Laagrisicopatiënten worden hoog
risicopatiënten wanneer er:
Actief spuiten vanuit een arterie in
een zweerbed injectie adrenaline
Een zichtbaar verhoogd bloedvat of uiteenzettende stolsel
Bloed uit een varices.
- Chirurgie
o Beleid om vroege chirurgie te doen bij patiënten over de 60 verminderd de mortaliteit.
Gastrische en duodenum zweren moeten gebiopteerd worden voor helicobacter en
maligniteit.
- Distale GI-bloeding
o Behandeling is conservatief, verwachtend dat het bloeden zal stoppen. Wanneer het
bloeden doorgaat, is het belangrijk dat de oorzaak gevonden wordt.
o Bloedverlies door diverticulars en ischemische colitis zijn zelflimiterend.
o Onderzoek via colonscopie wanneer het bloeden is gestopt.
o Bloedverlies wat niet zo snel is maar wel terugkerend is vaak angiodysplasie. Hierbij
kan colonscopie diagnostisch en therapeutisch zijn.
o Wanneer colonscopie niet voldoende is kan er radio-isotoop scanning of
arteriografie (bij snelle bloedingen)
