Basale oncologieKanker: toename met leeftijd
Vaak: mammacarcinoom (borstkanker), prostaatcarcinoom, longcarcinoom, coloncarcinoom
Kankergenen (gemuteerde genen) = oncogenen
Tumor surpressor genen = produceren eiwitten die het ontstaan van kanker afremmen door te
zorgen voor een goede celcyclus en deling, ze bevorderen het herstel die ontstaan door het DNA en
zorgen er voor dat afwijkende cellen sterven
> Wanneer deze worden uitgeschakeld, vervalt het immuunsysteem en kan er eerder kanker
ontstaan
Kankercel fasen
- Initiatiefase: mutatie DNA
- Promotiefase: cel word verder ontwikkeld door kanker cel door invloeden van buiten af,
bijvoorbeeld een hormoon
- Progressiefase: kankercel gaat onomkeerbaar delen en begint te groeien/ uitzaaien
Oorzaak:
- Erfelijke aanleg
- Ongezonde leefstijl en/of blootstelling aan chemische stoffen
- Straling: kunnen in de codering stukken uitvallen > mutaties (atoombom) > fysische invloed
- Chronische ontsteking: colitis ulcerosa > langdurige irritatie weefsel kan kanker ontstaan
- Virale infecties: hepatitis B virus, HPV, HIV; kunnen verandering geven in het DNA
- Onbekende factoren
- Invloeden van buitenaf, hormonen
- Verminderde afweer (immunodeficiëntie): kan normaal kanker herkennen, door medicatie
kan afweer verminderd zijn, wordt kanker niet meer herkent
Begrippen:
- Tumor: zwelling kan goedaardig en kwaadaardig zijn
- Hypertrofie/-plasie:
o Trofie = vergroting weefsel zonder toename aantal cellen (spieren bijv.)
o Plasie = vergroting weefsel met toename aantal cellen (beenmerghyperplasie bij
zuurstofgebrek, prostaathyperplasie, lymfehyperplasie)
- Atrofie: vermindering grootte weefsel, bij weinig gebruiken spieren bijv. bij vagina/ uterus
door gebrek aan geslachtshormonen
- Metaplasie: weefsel overgaat in ene type op het andere, bij baarmoederhals 2 type epitheel
met duidelijke scheidingslijn, dit kan opschuiven door hormonen, kan kanker door ontstaan
- Dysplasie: vorming van abnormale cellen, soort voorstadium van kanker bij bijv.
baarmoederhalskanker
- Neoplasie: gezwel vorming door nieuwe cellen kan goed en kwaadaardig zijn
- Benigne/ maligne: B: goed M: kwaad
- (On)gedifferentieerd: mate waarin het weefsel lijkt op het weefsel waar de kanker ontstaan
is. On: lijkt helemaal niet meer op oude weefsel Goed: lijkt wel veel op oude weefsel
- Anaplasie: kankercellen lijken totaal niet meer op oude cellen waar de kanker ontstaan is
Metastasering (uitzaaien naar andere delen van het lichaam): lymfogeen/ hematogeen, kankercellen
worden via de lymfeklieren verspreid door het lichaam
