HOOFDSTUK 1 Introduction
Psychopathologie is de wetenschap die de ziekten van de geest/mentale ziekten bestudeert.
Developmental psychopathology = intense, frequent, and persistent maladaptive patterns
of emotion, cognition, and behavior considered within the context of normal development, re-
sulting in the current and potential impairment of infants, children, and adolescents.

Het is soms lastig om psychopathologie te begrijpen. Ouders, docenten en kinderen vragen
zich vaak af of bepaald gedrag echt een stoornis is of het slechts afwijkend gedrag is. Om op
deze vraag antwoord te geven is het belangrijk om de connecties te maken tussen typische
en atypische ontwikkeling. De meeste theorieën, onderzoekers en clinici benadrukken het
gebruik van een model waarin de typische ontwikkeling beschreven staat voor kinderen van
een specifieke leeftijd. Hierin worden de individuele, familiale, etnische, culturele en
maatschappelijke opvattingen over wenselijk en onwenselijk gedrag beschreven van

,kinderen en adolescenten. Tegen deze achtergrond is het makkelijker om kinderen te
identificeren die zich uitzonderlijk gedragen en functioneren.

Maar wat is typisch/normaal gedrag?
- Statistical deviance = the relative infrequency of certain emotions, cognitions, and/or
behaviors. Children who display too much or too little of any age-expected behavior might
have a disorder.
- Sociocultural norms = the beliefs and expectations of certain groups about what kinds
of emotions, cognitions, and/or behaviors are undesirable or unacceptable. Children who
fail to conform to age-related, gender-specific, or culture-relevant expectations might be
viewed as challenging, struggling, or disordered. Dit is dus niet in alle culturen etc.

- Mental health perspectives = theoretical or clinically based notions of distress and dys-
function. A child’s psychological well-being is the key consideration. Children who have a
negative quality of life, who function poorly, or who exhibit certain kinds of symptoms
might have a disorder.

Bij al deze drie definities van normale/typische ontwikkeling zijn waarden erg belangrijk. Er
moet onderscheid gemaakt worden tussen adaptation en maladaptation en de persoonlijke
of groepsstandaarden van poor, adequate/average, of optimal adaptation.
- Poor adaptation = what is considered not okey and acceptable.
- Adequate/average adaptation = what is considered okey, acceptable, or good enough.
- Optimal adaptation = what is excellent, superior, or “the best of what is possible”.
Het gaat hier dus om de manier waarop kinderen omgaan met moeilijke situaties. Sommige
hebben moeite en houden er moeite mee, andere bewegen zich wel richting de positieve
ontwikkeling en sommigen ontwikkelen zich, ondanks de moeite, zelfs optimaal.

Er zijn verschillende behoeften die ieder kind heeft. Deze kunnen ook gesteld worden als
typisch/normaal in de ontwikkeling:
- De behoefte aan veilige relaties.
- De behoefte aan fysieke bescherming, veiligheid en regelgeving.
- De behoefte aan ervaringen die zijn afgestemd op individuele verschillen.
- De behoefte aan ervaringen die passen bij de ontwikkeling.
- De behoefte aan het stellen van grenzen, structuur en verwachtingen.
- De behoefte aan stabiele, ondersteunende omgevingen en culturele continuïteit.
Preventies en interventies zijn vaak gebaseerd op deze behoeften.

We kunnen dus stellen dat het moeilijk is om definities te geven van atypisch gedrag.
Daardoor is het ook moeilijk om schattingen te maken van hoe veel kinderen atypische
gedrag vertonen. Bij het maken van een schatting van het aantal probleemgevallen bij
kinderen moet er rekening gehouden worden met persoonlijke, klinische en beleidsgevolgen.
Als een bepaald probleemgedrag bijvoorbeeld extreem vaak voorkomt, moet mogelijk het
schoolprogramma hierop aangepast worden.

- Developmental epidemiology = frequencies and patterns of distribution of disorders in
infants, children, and adolescents. Deze tak van de wetenschap houdt zich niet bezig met
het diagnosticeren of behandelen van problemen, maar enkel hoe vaak probleemgedrag
voorkomt en waar dit vandaan komt etc.
- Prevalence = all current cases of a type of disorder. It refers to the proportion of a popu-
lation with a disorder.

,- Incidence = new cases of a type of disorder in a give time period. It refers to the rate at
which new cases arise.
De prevalentie van een bepaalde stoornis bepalen kan door een random sample in een grote
populatie of door samplen op scholen (beoordeling van docenten). Het maakt niet uit welke
manier gebruikt wordt, duidelijk is wel dat veel kinderen problemen hebben. 13% van de
kinderen tussen 8 en 15 jaar hebben te maken met een stoornis.

Slechts 50% van de kinderen met problemen krijgen daar daadwerkelijk hulp bij.
Barriers to care = factors that impede access to mental health services.
- Structural barriers such as lack of provider availability, inconveniently located services,
transportation difficulties, inability to pay, inadequate insurance coverage, or both.
- Individual barriers such as denial of problems or lack of trust in the system.
- Sociocultural barriers such as the stigma of psychopathology or mental illness.
Om dit te verbeteren moet er meer gelet worden op preventieve zorg bij risicogroepen,
toegankelijkheid van mentale zorg voor iedereen en de mentale gezondheid moet een
belangrijker punt zijn in ontwikkelingsprogramma’s.

In arme landen worden de kans op mentale problemen bij kinderen vergroot door een gebrek
aan goede gezondheid en onderwijs. Verder is de bewustwording van mentale ziekten
schaars en is er weinig tot geen zorg. In veel van deze landen zijn er kinderen die te maken
hebben met trauma door hongersnood, oorlog etc. Interventies om deze kinderen te helpen
moeten preventief ingrijpen en behandeling leveren om de kinderen te helpen. “Far too many
children experience displacement, hardship and loss; the negative impact on physical and
psychological well-being is enormous.”

Stigmatization = negative attitudes (such as blaming or overconcern with dangerousness),
emotions (such as shame, fear, or pity), and behaviors (such as ridicule or isolation) related
to psychopathology and mental illness.

Het begrijpen van de ontwikkeling en behandeling van psychopathologie is niet voldoende.
Dit is slechts de helft van het probleem. De rest van het probleem zit in stigmatisatie van
mentale problematiek waar mensen mee te maken hebben. We moeten onze tolerantie voor
diverse groepen met problemen verhogen en hen op alle vlakken hulp aanbieden.

HOOFDSTUK 2 Models of child development
Models of development, psychopathology, and treatment allow us to organize our clinical ob-
servations of children and our research findings into coherent informative accounts. In this
chapter, historical models that have contributed valuable ideas to our contemporary under-
standing are summarized.
- Dimensional models of psychopathology = models that emphasize the ways in which
typical feelings, thoughts, and behaviors gradually become more serious problems, which
then may intensify and become clinically diagnosable disorders (continuous, quantita-
tive). Geen scherpe grens tussen typisch en atypische gedrag.
- Categorical models of psychopathology = models that emphasize discrete and quali-
tative differences in individual patterns of emotion, cognition, and behavior (discontinu-
ous, qualitative). Scherp onderscheid tussen typisch en atypisch gedrag.

Physiological models = models of psychopathology that emphasize biological processes,
such as genes and neurological systems, as being at the core of human experience. Physio-
logical models explain the development of psychopathology, its course, and its treatment in

, terms of biological factors. We kunnen hierbij denken aan gen-omgeving-interactie, genen,
hersenstructuren etc. Om psychopathologie waar te nemen, vergelijken we deze factoren
tussen kinderen (waar ze overeenkomen en waar ze duidelijk verschillen).
- Connectome = the diagram of the brain’s neural connections. Een connectoom is een
complete kaart van de neurale verbindingen in de hersenen. Deze tak van wetenschap
kijkt dus naar connecties tussen hersengebieden en de functies die daarbij horen.
o Node = knooppunt in de hersenen die gebieden met elkaar koppelt.
o Hub = een knooppunt in de hersenen die centraal staat en meerdere gebieden met
elkaar koppelt.
o Module = groepen van nodes met sterke (meerdere) onderlinge verbindingen.
- Sensitive (or critical) periods = een sensitieve periode is een periode waarin de omgeving
een permanent effect kan hebben op een bepaald onderdeel in de (hersen)ontwikkeling.
Voor de kritische periode is bepalend dat bij afwezigheid van bepaalde stimuli er een
negatief effect ontstaat op de (hersen)ontwikkeling.
- Vroeger werd gedacht dat de hersenen alleen invloed hebben op het gedrag van
kinderen (unidirectioneel); tegenwoordig weten we dat er sprake is van een bidirectionele
invloed (hersenen beïnvloeden gedrag, maar gedrag beïnvloedt hersenen ook).
- Neural plasticity = the ability of the brain to flexibly respond to physiological and envi-
ronmental challenges and insults. (Neuroplasticiteit = veranderingen in de hersenen als
gevolg van ontwikkeling, leren of ervaring).
- Some basic definitions related to genetics:
o Genotype = the genetic make-up of a cell, an organism, or and individual.
o Phenotype = the observable characteristics of an individual, influenced by genotype.
o Behavior genetics = the study of the joint effects of genes and environments.

o Molecular genetics = studies of the effects of specific genes at the DNA level.
o Genome-wide association studies = a research method that involves rapidly scan-
ning markers across the complete sets of DNA, or genomes, of many people to find
genetic variations associated with a particular phenotype (such as a disease or disor-
der). Deze methode wordt gebruikt om de invloed van genen te onderzoeken. Er
wordt naast deze methode ook gebruikt gemaakt van twin, family, en adoption
studies, genetic sequencing en screening.
o Heritability = the proportion of phenotypic differences among individuals that can be
attributed to genetic differences in a particular population.
o Gene-by-environment effects = correlations between genes and environments that
involve differential exposure to environments or experience. There are three types of
gene-by-environment effects: passive, active, and evocative correlations.
o Gene-by-environment interactions = the interactive effect between genetic and en-
vironmental factors, including the influence of genes on vulnerability to risk factors.
o Epigenetics = the effects of experience and environment on the regulation of gene
expression. The resultant changes in gene expression can be transmitted across
generations. Het gaat hierbij dus niet om de aanwezigheid van een gen, maar om de
activiteit. Genen kunnen aan- of uitgezet worden door bepaalde
ervaringen/omgevingen.
- Risk alleles = genetic variants that impair (=aantasten) general processes (e.g., cogni-
tive or emotion functions) across many disorders. Deze allelen spelen een grote rol in de
psychologische modellen van de psychopathologie.
- Polygenic models = an etiological model of disorders based on the cumulative and addi-
tive effect of multiple genes.
