Introduction/Scenario
Consider the scenario an employee has suffered an injury which has resulted in the employee taking time
off work. As all accidents need to be reported under RIDDOR you need to take the necessary steps to
ensure that it is prevented, by completing the relevant forms.
Task 1 (P10)
It is important to report and record accidents and incidents. Explain in at least 200 words the underlying
principles involved. You should make reference to what records are kept, and for what purpose they are
used.
It is important to report and record accidents/incidents that happen in the workplace so that the
workplace always has a log of what has happened. There are many different types of records that must be
kept by companies because it is a legal requirement that they must record any incidents that take place
and record them to relevant governing bodies.
In all companies there will be a history log which will have all incidents that have taken place recorded, this
is beneficial to companies because it allows them to see trends in any certain incidents. From this they can
begin to distinguish how these incidents are re-occurring and start to put it down to things like; People not
listening to rules, incompetent workers whom lack skills in the certain task or it may even be that the
certain piece of equipment is inadequate and needs repairing.
Furthermore, by keeping record of all the different type of incidents that have taken place in the workplace
it gives the Health and Safety team the opportunity to update procedures in line with the HSE website to
ensure the workplace can run as safe as possible.
One of the most popular ways of reporting incidents and accidents that have taken place in the workplace
is RIDDOR (Reporting of Injuries, Diseases and Dangerous Occurrences Regulation 2013). RIDDOR is used to
keep workers safe at work and is a legislation which can hold employers responsible for any bad working
behaviours. The purpose of RIDDOR is so that the workplace is constantly enforcing safe working practices
to avoid anyone falling off the bandwagon and not adhering to health and safety measures that have been
put into place.
Task 2 (P11)
Using an engineering workplace of your choice: (ideally your place of employment)
Obtain an accident or incident report form. One from your own employer would be more
beneficial.
Complete this form for an accident involving yourself cutting a finger whilst removing swarf from a
lathe with your hand.
Consider the circumstances that led to this accident, and what actions should be taken to ensure it
does not happen again. Include this on your form in the appropriate section.
, Actions: This accident occurred due to the person brushing the swarf away from the Lathe without wearing
any gloves or appropriate PPE which could’ve prevented this from happening (equipment like a dust pan
and brush could’ve prevented this injury from occurring too). To ensure that this does not happen again,
gloves should be provided to the workers or they could buy a cleaning station which has the appropriate
cleaning equipment to remove swarf etc.
Task 3 (P11)
For the 4 scenarios below describe the actions to be carried out by the individual, a first aider and the H&S
department. (Not all 3 people will be involved in each scenario)
Injury = report to section leader, any injury that requires first aid you as individual would report the
incident to first aider, first aider would then take a look at injury and apply treatment – report in accident
book and first aid log (what happened, what treatment was applied), accident log would be looked at by
H&S and would be look at
A cut on the thumb whilst cleaning a work bench:
The individual will need to tell their cell leader that they have a cut. The cell leader will then get the
relevant first aid trained employee to have a look at the cut on the thumb. The first aider will then give the
appropriate treatment and will have to then record the incident into the first aid log. No further action will
be needed after this point.
A broken leg resulting from a slip on a wet floor:
In the incident of a broken leg resulting from a slip on wet floor the individual should shout for help and
get attention to themselves. The first aider should then come to assistance and assess the situation, they
would then call for an ambulance (the first aider should stay with the casualty until paramedics arrive).
Once the paramedics have arrived they will begin treatment on the patient and record the incident in the
accident book. From here, the Health and Safety team will have to report this incident through RIDDOR
due to the severity of the injury. This will then be published onto the HSE website.
