Hypofyse adenomen❖ hypofyse-bijnieras
➢ overproductie: ziekte van Cushing → centrale adipositas, glucose
tolerantie/diabetes, huidatrofie, virilisatie, osteoporose, psychische
stoornissen, moonface
■ diagnose:
● is er sprake van hypercortisolisme? 24-uurs excretie cortisol
urine, 1 mg dexamethason suppressietest overnacht,
middernacht (speeksel) cortisol.
● is het ACTH afhankelijk? ACTH serum
● hypofyse of ectopisch? lange i.v. 7 mg dexamethason
suppression of inferior sinus petrosus sampling
■ behandeling:
● transsfenoïdale resectie
● bilaterale adrenalectomie
● medicamenteus: metyrapon, ketoconazol, etomidaat
➢ onderproductie: ziekte van Addison → moeheid, algemene zwakte,
hyperpigmentatie, vermagering, hyponatriëmie, hyperkaliemie,
hypercalciemie, obstipatie, diarree, duizeligheid, eosinofilie, vitiligo
■ diagnose:
● insuline tolerantietest (gouden standaard)
● metyrapon test: remming cortisol zou via verhoging ACTH
moeten leiden tot toegenomen cortisolproductie
● CRH stimulatie test → verhoging ACTH → verhoging cortisol
● (ACTH stimulatie test = vals negatief wann. kort bestaand)
■ behandeling
● hydrocortison (3x/dd, korte T1/2): bij ziekte/stress verdubbelen!
● (prednison en dexamethason hebben lange T1/2 waardoor
makkelijker klachten cushing)
❖ prolactine #1 hormonaal actieve tumor!
➢ overproductie: galactorroe (vaak eenzijdig), libidoverlies, erectiele dysfunctie,
sec. amenorroe
■ diagnose: serum prolactine/MRI
■ behandeling:
● dopamine agonist (zeer effectief)
● behandeling hypogonadisme
● transsfenoïdale resectie
➢ onderproductie geeft geen klachten, behalve bij een vrouw in de
borstvoedingsperiode
❖ Groeihormoon - IGF 1
➢ overproductie: acromegalie of gigantisme (wanneer epifysairschijven nog niet
gesloten)
■ diagnose:
● insuline tolerantie test → stress → stijging GH
● GHRH argininetest moet ook leiden tot stijging GH
● verhoogd IGF 1 serum
■ behandeling:
● transsfenoïdale resectie
