In Huston’s (2010) brief, but impressive article (required reading this week), the author outlines several leadership competencies that EVERY nurse leader will need for 2020. That year is not too far in the future, is it? 1. Select one of the eight leadership competencies Huston described and relate it to your own leadership of nurses and nursing. This should promote a robust discussion as we come from different clinical and nonclinical perspectives. 2. Working on a short stay unit, which is exceptionally fast paced with discharges and admits. Our manager encourages us we embrace both competencies daily. These are reflective on patient safety, patient satisfaction and healthcare outcomes. Sometimes this can be a challenge due to the way one practices and/or personality conflicts. As Carol Houston (2010) states “Health in 2020 will be characterized by highly educated, multidisciplinary experts and this will complicate, not ease teamwork” (p.14). An example that I can relate to with the lack of collaboration/team building is when new medical residents come into the facility. I recently had a very demanding complex patient that was told that orders would be placed within fifteen minutes just as I was getting report. It had been a half an hour and no orders yet. I began paging the House PA to figure out who said that and why orders weren’t placed. House PA said it wasn’t their patient. I called the resident team, no response. Then called my shift administrator to help me figure out who was covering for the attending. After nearly an hour and a half, a resident called me back stating they would put some orders in but nothing for pain. The patient ripped me a new one several times. I asked that resident to come and see the patient. After another hour and a call to his senior resident, he finally showed up, angry at me. I told him, he needs to see the patient and explain the plan of care. He told me he wasn’t giving her anything, she’s drug seeking. Then after he came out, he gave her everything she wanted with a remark of “I’m giving what she wants until the morning team comes in, she is not my problem” and walked away. Effective communication would have prevented the whole situation. A BSN prepared nurse can aid the nurse leader by giving insight into areas that need to be addressed. As a bedside nurse, I feel that medication wasting whether in pill, IV push, IV fluids or IV piggy back is an area that needs to be addressed. We as nurses, administer a multitude of medications in various forms daily while at work. Wasting medication can become be very costly because the organization pays for the volume (Finkelman, 2016).
