II. Reason for Referral
Timothy is a fifteen-year-old male, who was referred for neuropsychological evaluation due
to a possible learning disorder or ADHD. The objective of the typical neuropsychological
evaluation is to draw inferences about the structural and functional characteristics of a
persons brain by evaluating an individuals behavior in defined stimulus response situations
(Cohen, Swedilk,& Sturman, 2013). Timothy has had average, but varying grades since the
first grade, going from B’s to D’s and back up to A’s. Timothy has experienced difficulties
with multistep tasks and often omits the important steps. Timothy often has difficulty in
organization of his homework assignments and study space. Other concerns noted are his
difficulties with grammar and vocabulary.
III. Current Symptoms/ Presenting Concerns
Timothy has a significant issue with behavioral problems such as following directions, being
loud and disruptive, and has authoritative issues. He is argumentative at school and in his
home. Timothy’s discord for following directions unless they are in his favor and is impulsive
at times. He displays lack of empathy, is self-centered in the aspect of always wanting to talk
about himself and always disrupting conversations without any lack of consideration.
Timothy hates getting in trouble but continues to defy authority at school and home. He
may try to harm himself or run away due to aspects of low self-esteem issues that have
