In health and social care there are agencies that help meet needs. Agencies need to work together in
order to help people stay healthy promoting a loner life expectancy and a better quality of life.
However, research has shown that in areas where there is high employment and issues with housing
and lack of transport then people tend to be unhealthy and a major problem in health care is the lack
of coordination between services. It is within their power to come together to improve the health of
service users so that they do not have to keep repeating their problems to different agencies over
again. If they work together it can consume time and help people get healthier faster. When services
didn’t work together there was very little progress with meeting individual needs making people
stressed and unaware on things that can help them. However, once the agencies started to work
together there has been a big improvement in health and social care as it has been more efficient and
cost effective.
In health and social care they use commissioning to fund their services. Commissioning is making
sure people have the right resources and facilities to suit their needs and to help people’s health by
funding health services like NHS. In commissioning there are three ways in which they fund them,
these organisations are; Clinical Commissioning Groups (CCGs), Local Health Boards, Health and
Social Care Board. The first organisation is CCGs. This was formed in 2013 by the NHS and its main
purpose is to provide funding for; health care services, provides services for the local area and
responsible for funding rehabilitate care, emergency care and mental health. This could benefit Nusrat
as due to her epilepsy she may have an unexpected attack therefore needing an ambulance. This
organisation would help Nusrat in this way because it would fund for that ambulance to bring Nusrat
to hospital to potentially save her life and perhaps to stop it from occurring again. The second
organisation is Local Health Boards in Wales. This is to provide people with care closer to their
homes, ideally in the same local area and they also help the service user with personalised care. This
could benefit Nusrat as she may want to live at home and because of this service it will allow her to
still attend her community centre for her future support and it will be beneficial to her because it will
be closer to her home. The third organisation is Health and Social Care Board. This organisation
allows constant needed improvements in health care. This could benefit Alice as she wants to improve
her health by having a healthier diet but she may not know how, the G.P. has referred her to a
nutritionist and by this service it has funded Alice seeing a nutritionist to help her health.
In the health care there is a health service called Clinical commissioning groups (CCG) this funds for
health care services to help the service user get better. This could link in to Martins case study as he
attends his G.P. for regular check-ups on his health and skin condition, the G.P. may run some tests
on his blood to make sure everything is okay and may examine his skin closer. This organisation
helps to minimise Martins challenge as it funds the facilities for this meaning Martin is being well
looked after.
In the Clinical commissioning groups there are organisations responsible for commissioning social
care services. These groups are set by the NHS to change behaviour and promote a healthier lifestyle.
This service funds to support promoting health and wellbeing by advertisements like; smoking,
attending schools and eating healthily. This would be beneficial to Alice as she is trying to maintain a
healthy life style. These advertisements minimises Alice’s challenge as it will make her aware of the
risks she is taking by drinking alcohol and if she is committed to have a healthy lifestyle then this may
boost her confidence to do so.

, In the clinical commissioning groups there are roles of bodies responsible for integrating health and
social care. This is when bodies aim to ensure that proper standards are maintained by health and
social care professionals and act when they are not. The Health and wellbeing board (HWB) is aimed
to; access the needs of their population, reduce inequalities in care, set out strategies for local
authorities (to improve the commission decision) and to encourage interrogation to achieve more
partnership. This will benefit Alice as she may need more antidepressants however die to the NHS
low funding and inability to help everyone then this will help Alice as it will need the needs of their
population by funding these antidepressants. This will boost her self-concept and self-worth as she
will feel as though the population is getting the right treatment for what they require and she will be
treated like an individual.
Common assessment framework
The common assessment framework is an intervention to assess a child’s needs. It identifies the needs
at an early stage and effects suggestions on how it needs to be met. This will benefit Nusrat as
professionals could identify that Nusrat has learning disabilities and struggles to communicate.
Professionals could see this and produce Makaton sheets so she has an equal chance at
communicating which could lead to wider things as in friendships, talking and understanding with
people which could benefit her as she won’t be so isolated now that she can have conversations with
people.
Care act 2014
The Care act 2014 aims to give a clearer, fairer care and support for the physical, mental and
emotional well-being for both the service provider and user needing the available care. This could
benefit Nusrat as it will ensure that Nusrats mother will be there and help make decisions for her care
plan and will support her whilst caring for Nusrat, it will meet the needs of the service user but also
make Nusrat feel secure knowing she can trust somebody.
Department of health
Department of health helps people to live a better life for longer, helps meet their needs and funds
health care. This would benefit Alice as she is an elderly lady. This policy would help meet her needs
because she would be provided with the correct facilities to help maintain a healthy lifestyle. This will
help her feel as though she is an individual and it’ll boost her self-esteem knowing she can improve
her health.
National framework for NHS continuing health care
The National framework provides a wide range of services like hospitals and residential care homes.
Its aim is to give a service provider the right and good care that is available regardless of wealth. This
could benefit Martin as he has financial issues. This would help him meet his needs because he will
receive the care he needs regardless of his health. This will make him feel happier and more confident
that he is not being discriminated due to his wealth. This way he can become healthier which may
encourage him to work again.
The education health and care plan
The education health and care plan is aimed for children and young people of ages up to 25 who needs
more support. It can be requested by parents, schools, doctors, health visitor, and nursery worker. If it
is thought that a child needs an EHC pan then the local authority will carry out an assessment. This
could benefit Nusrat as she has learning disabilities that affect her health. It will make her feel happier
and self-assured because she has been given an opportunity to help her education and care plan.
