Pierre, a 61 year old Angolan war veteran, had been experiencing Post traumatic stress
disorder (PTSD) symptoms for more than 40 years. During the years he frequently
averted images and thoughts related to witnessing his fellow service members being hit
by an improvised explosive device (IED) while driving an infantry fighting vehicle (IFV)
also better known as the Ratel IFV. Over the years, Pierre became progressively
depressed which resulted in psychological distress and begun abusing alcohol as an
escape goat to help ease his PTSD symptoms. He had complications in his employment
which led to an unstable predicament in his workmanship due to his frustrations and
anger with fellow employees. Despite his past failures, Pierre owns a small successful
gardening business, trying to take comfort in the fact that he still rose above his
distressing past. Striving for power is motivation for Pierre to move forward and deal
with his psychological distress and sufferings. He proclaimed feeling muddled and dazed
around his wife and daughter which resulted in multiple divorce cases with numerous
woman and an estranged relationship with his only child, Jolene. In addition to a
spectrum of other PTSD symptoms, Pierre had persistent nightmares of the incident in
which he was the leading driver of a convoy. As their IFV was hit, the explosion ripped
Pierre out of the window, leaving him behind in the field alive, while his whole team
went up in flames. He recalled trying to pull some of his service members out of the
burning IFV, but alas everyone as well as his best friend Jake, died that day. Pierre tries
to ease his guilt feelings of that event by giving back to the community through
volunteering but still he feels that it would never be enough and nothing would ever
bring his fellow members back.
After a comprehensive assessment of his PTSD and co-grim symptoms, psycho-
education about PTSD symptoms and a rationale for utilizing trauma concentrated
cognitive interventions; Pierre received 12 sessions of cognitive processing therapy for
PTSD. He was first appointed cognitive worksheets to begin self-monitoring events
regarding his thoughts about these events and his ensuing emotions. These worksheets
were used to sharpen Pierre to the types of cognition that he was experiencing about
current day events and to valuation that he had about the explosion and the burning
infantry fighting vehicle. One of the thoughts he recorded associated to the explosion
was “If only I tried harder to evacuate them; I should have pulled through for them,
but I didn’t” Pierre recorded his related feelings to be regret and a source of guilt. The
worksheets were used as an opening point for captivating in a Socratic dialogue.
