NR602: Primary Care of the Childbearing and Childrearing Family.Patient Information: LH, 7 yrs old. Caucasian male (S) Subjective: CC: Difficulty breathing, barking cough and nasal congestion X 3 days HPI: L.H is a 7 yr. old male who presents to the office with his parents for a follow-up appointment. Mother notes that patient was seen in our office three days ago in our office for URI. For the three days the patient has had a decrease in appetite and fluid intake. Mom reports the LH is not able to sleep at night due to cough and he says he has trouble breathing. Mom reports that the patient's cough has become more productive and yesterday L.H. developed a fever of 101.5. Mom states that she has given LH the antibiotic as prescribed. Mom notes that patient has been afebrile today with no s/s of respiratory distress. Mom notes that she has been using a cool mist vaporizer in patient's room at night time. No recent travel or sick household contacts, patient is up to date on his immunizations. Current Medications: Amoxicillin chewable 250 mg 2 tabs BID X 10 days Allergies: No known allergies. PMHx: Birth History • Birth Length: 50.5 cm (19.88") Weight: 7 lb 2.1 oz (3.235 kg) • Delivery Method: Vaginal, Spontaneous Delivery • Gestation Age: 39 3/7 wks PSHx: None.
