Pharmacotherapy for Cardiovascular Disorders Case Study

The purpose of this case study is to address how age can influence pharmacokinetic and

pharmacodynamic processes. The case study presented is “Patient AO has a history of obesity

and has recently gained 9 pounds. The patient has been diagnosed with hypertension and

hyperlipidemia. Drugs currently prescribed include atenolol 12.5mg daily, doxazosin 8mg daily,

hydralazine 10mg QID, sertraline 25mg daily and simvastatin 80mg daily.” Implications of age-

related changes in relation to drug therapy processes are explored. Lastly, recommendations for

improving AO’s drug therapy plan are made with an explanation of recommendations.



Age Affecting Pharmacokinetics and Pharmacodynamics

Age is a significant factor that has the potential to influence AO’s pharmacokinetic and

pharmacodynamic processes with drug therapy. Pharmacokinetics studies the basic processes of

absorption, distribution, metabolism, and excretion while pharmacodynamics studies the effects

that drugs have on the body (Rosenthal & Burchum, 2021). In the aging body there are changes

in all the pharmacokinetic processes. Decreases in small-bowel surface area and slowed gastric

emptying can affect absorption (Ruscin & Linnebur, 2018). Distribution can be affected in the

aging malnourished patient when there is a decrease in serum albumin. With this decrease of

protein, the drug effects are enhanced because there are higher concentrations that are not bound

to albumin which can cause toxic levels (Ruscin & Linnebur, 2018).

Unfortunately, pharmacodynamics does not have as much data to support age related

changes. According to Sera and Uritsky (2016) “differences in pharmacodynamics between

young and older adults may be due partly to age-related changes and partly to changes in
