package of care for the individuals.
Multi-disciplinary working means working as part of a team made up of health and social care
professionals from different health and social care services, for example community social services
may work with the mental health organisations (such as Mind) to help service users with mental
health problems within the local community, for example Brenda would benefit from these multi-
disciplinary teams. These teams are very important as it ensures coordinated care because all of
the professionals work together, emphasising the need for effective communication and planning.
By doing this, this means that there are no role / responsibility duplications making the service users
time more valuable.
Different agencies and professionals need to work together so that the needs of service users are
met as well as to provide a seamless response to individuals with multiple and complex needs. Each
agency can be linked to at least one aspect of health and social care, there are some commission
agencies that affect healthcare and other’s social care service.
There are many organisations responsible for commissioning health and social care services, within
the United Kingdom there are three main bodies; the Clinical Commissioning Groups (CCGs), Local
Health Boards in Wales and Health and Social Care Board in Northern Ireland.
The Local Health Boards within Berkshire for example, are formed to redesign the delivery of the
NHS. This would mean that care can be delivered more effectively, as a result this can lead to
