No structural or biochemical marker for IBS, symptom based diagnosis.

Any of the following symptoms in the past six months plus:

 Abdominal pain or discomfort
 Bloating
 Change in bowel habit

Plus at least two of:

 Symptoms made worse by eating
 Passage of mucous
 Abdominal bloating
 Altered passage of faeces e.g. straining, urgency etc.

Further investigation for red flag:

 Unexplained weight loss
 Rectal bleeding
 FH of ovarian or bowel cancer
 Loser stools for > 6 weeks in > 60

Pathophysiology:

 Genetics and environment
 Abnormal gastrointestinal motility
 Visceral hypersensitivity and the brain gut axis
 Psychological dysfunction
 Microscopic inflammation

Gas displacement in healthy patients is not passive its actively propelled
independent of liquids and solids. IBS patients have impaired gas handling.
Gas pooling may cause pain.


Treatment is dietary and lifestyle changes including: Reduce insoluble fibre,
caffeine, chewing gum, Low Fermentable oligo / di and monosaccharides
(FODMAP) diet.
