goals and outcoies. There can also be integrated care pathways, which are structured
iult-disciplinary care plans that detail essental steps in the care of patents with a
partcular clinical issue. They encourage the translaton of natonal guidelines into local
protocols and their applicaton to clinical practce. They also iiprove systeiatc collecton
and abstracton of clinical data for audit and for proiotng change in practce. Integrated
care plans exist for over 45 conditons or procedures and natonal users’ groups exist in
order to ofer advice and support in their use. They aii to help coiiunicaton with
patents through allowing thei access to a clearly writen suiiary of their antcipated care
