insufficient testing caused system failure.In this case study the Therac-25 was

never tested with the software and hardware hence the system failure and hence

causing radiation overdoses to the patients. The second example is the AT&T the

telephone networks failed and caused the telephone voice to get lost due to failure

of retesting the telecommunication switching program. The program had been

tested before implementation but not after the changes and this caused a software

error (Baase, 2018).


2. What was one cause in the delay in the completing of the Denver Airport.


Denver Airport completion delay was caused by mainly the computer-controlled

baggage-handling system. The project had gone through various specification

changes after the project began and this led to the failure in the ultra-modern

luggage carrier system. Also there was inadequate development and testing of the

system prior to its implementation. The time that was allocated for testing was

insufficient thereby errors with the program could not be detected (Baase, 2018).
