- 24-jarige vrouw, 2e trimester, infectie bovenste luchtwegen, antibioticum doxycycline →
tijdens 2e trimester tanden ontwikkeld, contra-indicatie: kans op aangeboren afwijkingen →
ander antibiotica
- vrouw na de bevalling depressief, gebruikt fluoxetine (prozac), wilt wel borstvoeding geven →
lage dosering geven of een ander anti-depressivum
De weg van het geneesmiddel:
- begint laag, de pil ingenomen, concentraties omhoog en daarna omlaag
- 1. absorptie: innemen door de mond (oraal), in het het bloed door de darm opgenomen
- 2. distributie verdeeld worden via het bloed, op de plaats van werking
- 3. metabolisme: lever
- 4. excretie: uitscheiding van medicatie → nieren en lever (gal door de lever uitgescheiden in de
darm)
ADME
Toedieningswegen- en vormen:
- Enteraal: oraal (tablet, dragee, capsule en drank) → per os/mond → maag passeren → in de
darm geabsorbeerd en rectaal (zetpil, rectiole en klysma)
- Transdermaal: (pleister) → 1x in de 3 weken, gedurende 3 wk geneesmiddel afgeven
- Topicaal: lokaal (inhalatie, zalf, lotion en IUD) → spiraal: progestageen= hormoon afgeven →
langere periode afgeven
- Parenteraal: niet via de darmen, injectie (subcutaan, intramusculair en intraveneus) → de pil:
depofera → progesteron in de spier injecteren= anticonseptie voor 3 maanden
