Upon initial assessment it was concerning that Brenda was going into premature labor. It was also concerning that she did not have a GBS result and that her baby had a higher heart rate than normal. PCC Brenda Patton stated that she would like to labor free of medication. What are some techniques that could be used to help her with her pain? There are many ways to help with pain with a medication free labor. The individual can get into a birthing tub, change positions, use a birthing ball, deep breathing, guided imagery, and music therapy. T&C What key elements would you include in the handoff report for this patient? Consider the situation-background-assessment-recommendation (SBAR) format. Some key assessments I would include in my handoff report would be she is a 18 year-old female G1P0 at 36 2/7 gestation and an allergy to penicillin. I would share her last vitals and any abnormal assessment on the head to toe. I would then let them know she had an unknown GBS status, a fever, and nausea. I would then tell them the three medications that were used to address these issues and lastly that she wishes to have a medication free labor. Quality Improvement (QI), Safety (S), and Informatics (I). To improve Brenda’s care, I would provide more teaching to her and her family about GBS when her results are back. I would also provide resources to her since she is having a teen pregnancy and may not be prepared. Also giving her lots of pain management skills to keep her wish of having a medication free birth but also teaching her that although she has that plan things may change. Concluding Questions Reflecting on Brenda Patton’s case, were there any actions you would do differently? Explain. During the simulation I forgot to ask about patient allergies before I administered the nausea medication. I only did that before the penicillin. I would have asked that earlier. I also should have notified the NICU that this baby might be delivered soon. Describe how you would apply the knowledge and skills that you obtained in Brenda Patton’s case to an actual patient care situation. I am now more prepared to work with clients who are at risk for having GBS bacteria. Also using all of the patient education about GBS, and pain management will be used in an actual patient care situation.
