ABDOMEN9 regio’s kennen, snel kunnen benoemen:
Rechter hypochondrium Epigastrische regio Linker hypochondrium
Rechter flank Umbilicale regio Linker flank
Rechter fossa Suprapubische regio Linker fossa
1. Patiënt komt binnen: naam en geboortedatum controleren
a. Zeggen dat pt zich mag uitkleden: tshirt uit, ondergoed mag aanblijven,
broek uit of losknopen zodat ook de liezen zichtbaar zijn
b. Ondertussen handen en stethoscoop ontsmetten
2. Patiënt verzoeken om te gaan liggen op de tafel, armen naast de zij te leggen
(zodat spieren niet aangespannen zijn)
a. Rechts naast de patiënt staan, dan sta je al aan de juiste kant voor
onderzoek van de lever
b. Voor je de patiënt aanraakt vragen waar hij/zij pijn heeft dit steeds
als laatste bij percussie en palpatie zodat onderzoek niet beïnvloed
wordt door de pijn
3. Algemene inspectie:
a. Algemeen nutritionele status, turgor (huidplooi), spiermassa inschatten,
obese pt of niet
4. Inspectie: alles benoemen wat je controleert of enkel zeggen wat je zou
noteren in het dossier
a. Symmetrie: ergens meer opgezet?
b. Tatoeages en piercings belangrijk in het kader van hepatitis B en C
c. Navel op een normale plaats gelokaliseerd? Beharing?
d. Aanwezigheid van littekens, striae, huidletsels, hematomen, huiduitslag
i. Inschatten in welk kwadrant om in het dossier te noteren
e. Kijken of je massa’s ziet, zichtbare peristaltiek, pulsaties
i. Duidelijk te zien bij een heel magere pt: gewone darmperistaltiek
en aortapulsaties
f. Kijken naar eventuele aanwezigheid liesbreuken in rust
5. Patiënt valsalva manoeuvre laten doen om een eventuele liesbreuk op te
sporen ter hoogte van:
a. Navel
b. Litteken
c. Inguinaal
d. Femoraal
6. Auscultatie: met membraan van uw stethoscoop, zorgen dat die op
kamertemperatuur is; als je dit pas na palpatie doet ga je de darmen te veel
activeren door het onderzoek
a. Op een random plek ausculteren, op diezelfde plek blijven gedurende
maximum 2 minuten, horen of er darmperistaltiek aanwezig is
i. Anders suggestief voor een paralytische ileus
ii. Indien je fles- en plasgeluiden hoort is dat suggestief voor een
obstructie
b. Aorta: stethoscoop halverwege tussen xyphoid en navel
