NR 511 Week 3: Clinical Case Study Part One Discussion Good evening professor and class. A 19-year-old male patient arrives to the clinic with a chief complaint of bilateral eye discomfort. Patient reports a feeling of sand in his eyes. He also complains of sensation feeling gritty in his bilateral eyes. The onset of symptoms started 2 to 3 days ago. He describes the pain as feeling moderate and rates his pain 2/10. He denies both aggravating and relieving factors at this time. However, he reports using OTC Visine drops yesterday that provided no relief of sensations but was effective in diminishing the redness. Three differential diagnosis for chief complaint Allergic Conjunctivitis-is caused from allergens. The mucosa conjunctival contains mast cells and IgE which are antibodies that are attached to the cell surface by receptors. The host then is revealed to the environmental allergen. This causes allergens to bind together with the IgE and the mast cell to cause an intertangle of molecules and transmission after degranulation of the cells. In return, inflammatory intermediaries cause the clinical manifestation, (Mashige, P., 2017). 1) Allergic Conjunctivitis 2) Allergic Rhinitis 3) Viral Conjunctivitis 2) Allergic Rhinitis – exposure to an allergen, dendritic cells, known as antigen-presenting cells (APCs). These cells are located in the mucosal surface. This causes allergens and peptides to build and form Class 11 allergen molecules, and, major histocompatibility complex (MHC). Both the antigen compound and class II molecule are bands that operate as T-cell receptors on the Naïve CD4+ T cells. This then results in the cells dividing and changing their functional type of Naïve CD4+ T cell to T helper (TH2) cells that are allergen specific. Cytokines produced by the initiation of TH2 cells encourage altering B lymphocyte cells to produce precise IgE and a quick upsurge of eosinophils, neutrophils, and mast cells. IgE antigen-specific connects with IgE receptors on mast cells or basophils, causing allergen-induced sensitization and infection or inflammation (Mashige, P., 2017). 3) Viral Conjunctivitis – Viral Conjunctivitis takes place underneath the epithelial layer. This is where the vulnerable host takes place beneath the epithelial layer. This includes a germinal center (GCs) which causes a swift assemblies of infantile lymphocytes that is connected by a sphere to established lymphocytes and plasma cells, usually found in the section of the inferior palpebral and forniceal conjunctiva , (Rosa, M., Lionetti, E., Reibaldi, M., Russo, A., Leonardi, S., Tomarchio, S., Avitabile, T., Reibaldi, A., 2015) Subjective/objective findings 1) Allergic Conjunctivitis
