Unit 3: RUA Topic Search Strategy Chamberlain College of Nursing NR449 Evidence Based Practice RUA: Topic Search Strategy Clinical Question In today's intricate healthcare system, providing pati ent information during shift report to the oncoming nurse, is a time of great liability and risk. Effective and accurate communication can improve overall quality of care for patients and reduce the amount of errors and injuries. According to Johns Hopkins Medicine (2016), medical errors and injuries are the third leading cause of death in the U.S. Improving patient safety trends by performing shift report at bedside can reduce the number of medical errors and injury in the healthcare setting. Including patients in the plan of care process will help them gain knowledge about their treatment and improve overall satisfaction rating. The problem in the healthcare setting today, is noncompliance to bedside shift reporting due to certain barriers within the hospital setting resulting in miscommunication. Lack of consistency and enforcement of a standardized method for hand-off report increases potential error when providing care. The Joint Commission estimated 65% of sentinel events were a result of miscommunication, especially during hand-off shift report (Cornell, Townsend-Gervis, Yates, & Vardaman, 2013). To assist in conducting research on this issue, PICOT will formulate clinical questions and answers. The population would be patients’ and nurses that participate in bedside shift report. The intervention would be performing change of shift hand-off in patients’ room. The comparison is between bedside shift report and traditional report not in the patients’ room. The outcome is to improve quality of care and improve safety for patients. Time- The length of time needed to preform accurate hand off report at bedside. My PICOT question is ‘Does implementing bedside shift report influence the outcome of patient satisfaction and safety?’ The purpose of this paper is to identify the effects involving patients when bedside shift report is used. Levels of Evidence To continue research for this project, it is important to find information that is supported by evidence. I used the mixed method to conduct my research which includes both quantitative and qualitative designs. Quantitative design will be used to examine the cause and effect relationship between the two variables, bedside shift report and traditional hand-off report using statistical evidence (Houser, 2018). Qualitative design will be used when collecting information directly through patient experiences. The question asked during this research was "What are the effects of patient satisfaction and safety when bedside shift report is not implemented?" The literature reviewed for this project supports that bedside shift report is beneficial for both nurse and patient. Search Strategy The topics I searched while finding an article for this project included "Improving care through bedside shift report", "Patient satisfaction and safety when using bedside shift report", and "Bedside shift report vs. traditional hand-off report." Initially, I used PubMed to conduct my research and eventually found both articles on the Chamberlain Library. When I first began conducting my research, I used the topic "bedside shift report" resulting in an overwhelming number of articles. I then searched a more specific topic within the Chamberlain Library to make the article selection easier. To narrow down the search results, I used the publication date filter to find and article published within the last five years and selected peer-reviewed articles. I also used the publication type filter to set my results to nursing based articles. The two most relevant articles that will provide guidance for my next paper and the groups work are Evaluation of bedside shift report: A research and evidence-based practice initiative by Victoria Schirm, Geneva Banz, Caitlin Swartz, and Marva Richmond and Effects of Multi-method Intervention on Bedside Report Compliance and Patient Satisfaction by Daniela Faloon, Holly Hampe, and Thomas Cline. In the article Evaluation of bedside shift report, evaluates nurses' perceptions regarding bedside shift report as it was being implemented in a hospital unit. The article Effects of Multi-method Intervention on Bedside Report Compliance and Patient Satisfaction used HCAHPS scores to evaluate how bedside shift report impacts patient satisfaction. In this study, patient noted a significant increase in patient involvement and being informed in the plan of care. I selected these articles to have one that was nurse-based and one that was patient-based. To determine the effects of bedside shift report, information regarding both patient and nurse is needed. Conclusion To obtain relevant research information and articles for the group project, we can start by creating PICOT questions. PICOT questions can help us focus our research on the problem of medical error and injury due to not complying with bedside shift report.
