TH 28 : SPINAL CORD INJURIES (13/12)INTRODUCTION
DEFINITIE
Spinal Cord Injury = schade of trauma aan het ruggenmerg met als resultaat verlies of impaired functionaliteit die
verminderde mobiliteit of sensibiliteit veroorzaakt. (paraplegie vs tetra of quadriplegie)
ETIOLOGIE
Ongeveer een gelijke verdeling van de nieuwe 200 patienten over vlaanderen en wallonie, over tetra- en paraplegie
en over complete en incomplete injuries met een kleine tendens meer naar tetraplegie en incomplete laesies. Vaker
veroorzaakt door trauma (vallen, verkeer, ..) dan door andere oorzaken.
LOSS OF FUNCTION
Verlies van motorische functie -> spieren die aangedaan zijn zijn voorspelbaar dankzij het innervatieniveau/myotomen
(key muscles)
Verlies van sensorische functie -> verschillende soorten sensibiliteit die niet altijd op eenzelfde manier affected zijn ->
in kaart te brengen adhv dermatomen.
Verlies van autonome functie -> craniale zenuwen zijn meestal nog intact, maar andere viscerale aansturing kan
aangedaan worden.
Bij een complete transsectie zal er zich een volledig verlies van functie voordoen. Bij een incomplete transsectie zal de
hoeveelheid van schade bepalend zijn en is kennis van de neuro anatomie belangrijk.
Posterior Cord Syndrome (eigen arterieën) verlies van proprioceptie, vibratiezin en touch
Anterior Cord Syndrome (eigen arterieën) verlies van motor function, pain en crude touch
Central cord lesion door sustainted pressure zorgt voor verlies van functie in de upper limb
Cauda equina (L2-L5) = lower motor neuron leasion gelijkende op een heel wijd verspreide perifere zenuw aandoening
CONSEQUENTIES
Respiratoir
Hoog risico voor : insufficiëntie, mucus productie,
obstructieve slaap apneu (OSA), atelectase,
pneumonia of bronchitis -> tweede meest
voorkomende doodsoorzaak
Spasticiteit
Motorische aandoening gekarakteriseerd door
snelheidsafhankelijke toename in tonische stretch
reflex tgv hyper exciteerbaarheid van de
stretchreflex (component van upper motor neuron
syndrome)
Aanwezig bij 70% van de patiënten door verlies van inhibitie zenuwbanen. Acuut een spinale shock fase.
Negatieve effecten: functioneel (moeilijkheden met transfers, pijn, ..), pijn, contracturen, decubitus, relaxatie en slaap
Positieve effecten: troficiteit, bloedperfusie, functioneel (voldoende rigiditeit kan helpen met transfers)
