KWAADAARDIGE HEMATOLOGISCHE AANDOENINGENINDELING • Leukemieën
o Kwaadaardigheid uitgaande van een hematopoietische cel van BM
• Lymfomen
o Kwaadaardige tumoren van lymfoïde organen. Hodgking-lymfomen (HL) vs niet-Hodgkin lymfomen (NHL)
• Myelomen
o Proliferatie plasmacellen
• Myelodysplastische syndromen
o Verworven (pre-)neoplastische aandoeningen → kwalitatieve en kwantitatieve afwijkingen van de reeksen, ineffectieve hematopoiese en
beenmerinsufficiëntie
• Myeloproliferatieve neoplasieën = Monoclonale ziekten uitgaande van hematopoietische stamcel met trage proliferatie die granulocytaire,
monocytaire, erytroïde en megakaryocytaire reeks inneemt

EIGENSCHAPPEN • Monoclonaliteit
o Dwz uitgaande van één cel
• Clonale progressie
o Kwaadaardige cloon blijft groeien
• Clonale dominantie
o Kwaadaardige cloon heeft proliferatief voordeel tov normale cellen
• Onderdrukking van normale clonen
• Genetische instabiliteit
o Clonen vertonen chromosale en/of genetische afwijkingen. Hebben tendens om in kwaadaardigheid toe te nemen
KLINISCHE MANIFESTATIES Door de gevolgen van clonale progressie, clonale dominantie en onderdrukking van normale clonen:
• Beenmerginsufficiëntie
o → anemie, granulocytopenie, trombocytopenie
o Niet in beginfase (hier eerder polycythemie, trombocytose en/of neutrofilie
• Orgaaninfiltratie
o Obstructieverschijnselen tgv retroperitoneale lymfadenopatie
o Pijn tgv rekking orgaankapsels, rekking endost en periost en compressie van zenuwwortsels
o Tekens van centraal zenuwstelselinvasie: intracraniële overdruk en/of meningeale prikkeling
• Immunologische afwijkingen
o Hypogammaglobulinemie tgv gestoorde synthese Ig → toename van infecties; vnl bij lymfoïde neoplasieën, B-cel NHL en CLL
o ↑auto-antistoffen → auto-immune anemie of trombocytopenie
o Gestoorde cellulaire immuniteit geassocieerd met toename infecties (virale, mycobacter en protozoair); vnl bij B-én T-cel NHL en HL
• Effecten van tumorcelproducten
o ↑urinezuurspiegels tgv celafbraak → uraatnefropathie en jicht (allopurinol geven)
o Osteoporose, ostolytische letsels, pathologische fracturen, hypercalciëmie tgv cytokineprouctie die osteoclasten activeert; vnl bij myeloom
, o Hyperviscositeitssyndroom: belemmering van circulatie tgv toename viscositeit agv
▪ Paraproteïne (monoklonaal Ig) bv bij macroglobulinemie of ziekte van Waldenström
▪ Polycythemia vera tgv toename RBC massa
▪ Leukostase of hyperleukocytosesundroom tgv sterke toename WBC bij AL of CML
HYPERVISCOSITEITSSYNDROOM:
• Kan leiden tot
o CZS symptomen (duizeligheid, syncope, coma, dood,..)
o Zichtstoornissen met dilatatie van netvliesbledvaten, netvliesbloedingen en papiloedeem
o Dyspneu tgv overvulling en congestief HF
o Abnormale bloedingen
• Behandeling → oorzakelijke factor verminderen:
o Bij Waldenström: plasmaferese
o Bij leukostase: leukoferese
o Bij polycythemia vera: aderlating
ONTSTAANSMECHANISMEN Theorie over moleculaire basis van kanker:
• Proto-oncogen → oncogen
• Tumorsupressorgen(en) onderdrukkt
Mechanismen om proto-oncogenen tot oncogenen te activeren:
1. CHROMOSOMALE TRANSLOCATIES EN HERSCHIKKINGEN
• Verworven chromosomale afwijkingen
• Stukken chromosomen afgebroken en naar andere afgebroken chromosoomstukken gebracht
• Aantal proto-oncogenen zijn niet verafgelegen van breekpunt → komen plots in de buurt van genetische sequentie die plots de expressie van die
oncogenen in gang zetten en onderhouden
• Voorbeelden
o Acute lymfoblastische leukemie van B-cel type of burkitt-type ALL:
▪ Altijd translocaties t(2;8), t(8;14) of t(8;22)
▪ Translocaties hebben gemeenschappelijk kenmerk: het c-myc-oncogen gelegen op chromosoom 8 + Igketen betrokken gelegen
op chromosoom 2(κ-lichte-keten-gen), 14(Ig-zware-keten-gen) of 22(λ-lichte-keten-gen)
▪ C-myc-oncogen komt door translocatie direct onder controle van genetische controlesequenties van immunnoglobulinegenen.
Deze controlesequenties (promotos en enhancers) zijn actief in B-cellen gespecialiseerd in Ig-genexpressie
o Chronische myeloide leukemie (CML)
▪ Gebalanceerde t(9;22)
▪ Veranderde chromosoom 22 = philadelphia chromosoom
▪ Het c-abl proto-oncogen van chromosoom 9 → naar BCR streek van chromosoom 22
▪ Tgv translocatie ontstaat er een nieuw chimerisch BCR/ABL fusieproteïne met sterkere tyrosinekinase activiteit dan die van het
ABL-genproduct ➔ cellen met t(9;22) translocatie hebben proliferatief voordeel tov cellen zonder translocatie
