Lengte 1.76, opname bij thuiszorg sinds 22-2-2018 tot heden. Zijn medische voorgeschiedenis is Dhr. Heeft
twee jaar geleden onder zijn huid een pacemaker gekregen. Dhr. Is bekend met een hoge bloeddruk, hij slikt
hiervoor azilsartan.
Gedragsfactoren van de zorgvrager:
Gedrag is niet te corrigeren, dhr. is tegen de medewerkers onvriendelijk, slikt zijn medicatie niet. Verwaarloosd
het huishouden en voedingsstijl.
Doel van de voorlichting:
Voorlichting waarom het innemen van de medicatie belangrijk is. ( arts en verpleegkundige)
- Uitleg over de voedingspatroon, dat het aangepast moet worden wegens in verband ongezond + de
darm aandoening die dhr. heeft. (diëtist en voedingsmedewerker)
- Psycholoog erbij, dhr. kan met emoties zitten die hem dwars zitten. Waardoor het gedrag veranderd is.
