IV - 25HÉMOPHILIES
DEFINITION
Maladies hémorragiques constitutionnelles liées à un déficit en FVIII (hémophilie A) ou en FIX (hémophilie B).
Mode de transmission : récessif lié à l'X ; seuls les garçons sont cliniquement atteints, les femmes sont conductrices.
ÉPIDEMIOLOGIE
- Environ 5000 hémophiles en France ; 1/3 des cas = pas d'ATCD familiaux → sporadique.
- 1/5000 naissances mâles pour l'hémophilie A contre 1/25.000 pour l'hémophilie B.
CLINIQUE
Grande hétérogénéité → sévérité de l'atteinte liée au taux du facteur déficient = corrélation entre taux et gravité clinique.
- Formes sévères : hématome et hémarthroses spontanées + hématuries, hémorragies gastro-intestinales, cérébro-
méningées….
- Formes modérées, atténuées : expression clinique moins bruyante, voire inexistante → risque hémorragique chirurgical.
ÉVOLUTION
- Fonction du degré de sévérité, du taux de facteur VIII ou IX.
o Si < 1% : hémophilie sévère → risque d’hémarthrose et de complications orthopédiques.
o Si 1 - 5% : hémophilie modérée → risque de complications faible.
o Si > 5% : hémophilie frustre → vie pratiquement normale.
- Mauvaise prise en charge :
o Saignements musculaires et articulaires → arthropathie hémophilique → handicaps.
o Hémorragies internes → risque mortel.
BIOLOGIE
• NFS : normale.
• Bilan d'hémostase :
o TS : normal
o TCA : ↑ (mais TCA(M+T) : N)
o TP : N
o Temps de Thrombine : N
• Dosage des facteurs XII, XI, IX, VIII → mesure de l'activité anticoagulante → permet de définir le type et la sévérité.
o Hémophilie sévère : FVIII ou FIX < 1% → 50% des cas !
o Hémophilie modérée : FVIII ou FIX entre 1-5% (→ 10-20% des cas). Forme identique chez tous les
o Hémophilie atténuée : FVIII ou FIX entre 6-35% (→ 30-40% des cas). individus d'une même famille.
• Dosage du facteur Willebrand (diagnostic différentiel en cas d'hémophilie A).
GENETIQUE
• Diagnostic des conductrices :
o Enquête génétique : établissement d'un arbre généalogique
- Conductrices obligatoires (portant X anormal) : filles d'hémophiles, femmes ayant eux 2 enfants hémophiles,
femmes ayant un enfant hémophilie et des ATCD familiaux.
- Conductrices "potentielles" : sœurs d'hémophile (50%), filles de conductrices obligatoires (50%), cousines
germaines d'hémophiles (12,5%)
