A/ Héparines
BUT
- Mesurer l'efficacité du traitement
- Évaluer le risque hémorragique
- Dépister les thrombopénies à l'héparine
MARQUEURS BIOLOGIQUES
Tests réalisés sur citrate de sodium :
• Tests chronométriques :
o Temps de céphaline + activateur.
o Temps de thrombine (= temps de coagulation d'un plasma citraté après addition de thrombine calcique).
• Mesure l'activité anti-Xa : "héparinémie" = reflet de l'activité anti-Xa de l'héparine circulant dans le plasma.
o Principe : mesure de l'action inhibitrice du complexe héparine-AT du malade vis-à-vis de FXa exogène.
o Détection : chromogénique → le FXa résiduel hydrolyse un substrat chromogénique → mesure variation d'absorbance.
o Résultats exprimés en UI/mL (HNF) ou en UI anti-Xa/mL (HBPM).
SURVEILLANCE D’UN TRAITEMENT PAR HNF
Bilan d'hémostase et numération plaquettaire avant tout traitement.
Premier contrôle dès la première injection
Surveillance ++ pour sujet âgé, insuffisance rénale, obèses, apparition d'un saignement
• Traitement curatif (contrôle quotidien quelle que soit la voie d'administration) :
o Prélèvement 4h après début perfusion IV, ou 4-6h après injection SC
o Plaquettes : 2 fois par semaine
o TCA : 1,5 à 3 fois le témoin
o TT > 120 secondes
o Anti-Xa : 0,3 - 0,6 UI/mL
• Traitement préventif (contrôle quotidien, puis 2 fois par semaine) :
o Prélèvement 4-6h après injection SC
o Plaquettes : 2 fois par semaine pendant 3 semaines, puis 1 fois par semaine
o TCA : 1 à 1,5 fois le témoin
o Anti-Xa : 0,1 - 0,3 UI/mL
SURVEILLANCE D’UNE HEPARINOTHERAPIE PAR HBPM
Bilan d'hémostase et numération plaquettaire avant tout traitement.
PAS DE SURVEILLANCE DE L'ACTIVITE ANTI-Xa NECESSAIRE
SURVEILLANCE PLAQUETTAIRE : 2 fois par semaines pendant 1 mois, puis 1 fois par semaine
Surveillance anti-Xa réservée aux cas particuliers : insuffisance rénale sévère, sujets âgés, saignements, poids extrêmes.
Seule l'activité anti-Xa est très bien corrélée avec le risque hémorragique.
TCA peu ou pas allongé par HBPM (sauf Tinzaparine - INNOHEP® à dose curative).
Pas de surveillance pour le Fondaparinux.
Traitement curatif (cas particuliers) : prélèvement 3-4h après injection, activité anti-Xa = 0,1-0,4 UI anti-Xa/mL
