coordination of care. Care coordination is an integral part of patient care. Its purpose is to ensure

that the needs of the patients are met and the integrated care services provided (Hannigan et al.,

2018). As nurses, we get the opportunity to care for millions of individuals that have complex

issues of health and social needs. This calls for the need for care coordination in order to meet

such needs. Coordination of care tends to synchronize health care delivery of patients from the

different health care professionals and specialists. The aim of doing this is to enhance health

outcomes of the patients and also help in reducing the costs of healthcare. Care coordination

should be conducted throughout the process of care, from arrival or access to the care, to

evaluation, diagnosis, development of a care plan and finally to follow-up or post care (NEJM

Catalyst, 2018). This presentation focuses on different aspects of care coordination including

effective strategies for collaborating with patients and their families, aspects of change

management, rationale for coordinated care plans based on ethical decision making, potential

impact of specific health care policy provisions and the vital role of nurses in coordination and

continuum of care. This knowledge will help prepare every nurse for effective care collaboration

in future.

Fundamental Principles of Care Coordination

Care coordination needs to be successful in order to have a positive impact on patient

care. This means that its elements and principles need to be applied. One of the elements or

principles of care coordination is ease of access to varieties of health care services and

professionals or providers (NEJM Catalyst, 2018). Care coordination cannot be effected without
