Enkel X-ray enkel nodig als er pijn is rond de malleolus en:
- Botgevoeligheid posterieur of punt van de laterale malleolus. Of
- Botgevoeligheid posterieur of punt van de mediale malleolus. Of
- Het onvermogen om direct 4 stappen te zetten
Voet X-ray enkel nodig als er pijn is in de middenvoetzone en:
- Botgevoeligheid tuberositas metatarsaal V. Of
- Botgevoeligheid os naviculare. Of
- Het onvermogen om direct 4 stappen te zetten
- Palpatie malleolus laterlis: posterieure top en distaal 6cm.
- Palpatie malleolus medialis: posterieure top en distale 6cm.
- Palpatie tuberositas metatarsaal V.
- Palpatie tuberositas os naviculare (ti bant insertie).
EBP:
- Toegepast om fractuur enkel/ middenvoet posttrauma <7d uit te sluiten.
- Fractuur uitgesloten indien patiënt 2 stappen kan nemen op aangedane voet en er geen
drukpijn op fibula, tibia, os naviculare en tuberositas metatarsaal V is en dus geen RX foto
geïndiceerd.
- LR- = ,08 (8 op 100 toch fractuur ondanks negatief resultaat)
Thompson test
Als calf squeeze test + (korte plantair flexie is afwezig tijdens knijpen): A-pees ruptuur
LR+= 13 LR-= .04

Kijken naar stand van calcaneï.
Vermoede van bottrauma of fractuur
vaststellen.
Kijken of patiënt zijn/ haar been
volledig kan steunen.
Kijken naar hypertrofie (groei) (kan wijzen op overtraining of
compartimentsyndroom).
Kijken naar vorm en configuratie achilles pees (normaal, dun, smal,
bkeen, wit/ tendinitis; verdikt/ ruptuur = gaping).
Kijken naar spieromvang kuit musculatuur. (verminderd door langdurige immobilisatie)
Kijken naar lumbale wk (lordose, abnormale scheefstand) (sagittaal en frontaal bekijken)
Kijken naar verkleuring of zwelling thv kuitmusculatuur en rond mal lat.
Kijken naar ant zijde van de spronggewrichten 🡪 zwelling = artritis.
Palpatie om te provoceren: bvb OAR bij vermoeden fractuur.
Palpatie om een kwaliteit van een beweging te evalueren:
bvb. Ant transl van tibia VKB letsel (Lachman test).
Palpatie om bv. De kuistpier, achillerspees te testen: bv.: crepitaties, onderbreking (gapping).
LR+ drukt uit hoeveelkeer de kans groter is dat de patiënt de ziekte/ het letsel eheft bij een positieve
test dan de ziekte/ het letsel dan toch niet te hebben bij een positieve test.
Best zo hoog mogelijk >10
LR- drukt uit hoe groot de kans is dat de patiënt ondanks een negatieve test de ziekte/ het letsel
toch heeft in vgl met de situatie dat de patiënt de ziekte/ het letsel werkelijk niet heeft bij een
