I am in the course NU 518 course 814. ____ do you give me consent to perform a cardiovascular and neurological assessment on, record it, and load it for my instructor for grading purposes?  Do you need to empty your bladder?  I will start by showing you four objects, remember them, and I will ask you to recall them later in the assessment: pen, watch, measuring tape, and reflex hammer. Assess Mental Status • Appearance and Behavior o Patient appears appropriately dressed for situation, age and lifestyle; hair, skin, teeth and nails appear well groomed. Patient’s facial expressions are appropriate with comfortable with eye contact. The patient is sitting in a relaxed position and behavior is cooperative and friendly. • Mood o Describe your mood for me? What is your overall mood on a daily basis? o Pts mood and affect are appropriate for the situation and setting and based on patients life events. • Judgement o What would you do if you found a wallet on the ground? Or What would you do if you were driving and a police car appeared behind you with the lights and sirens on? Assess Cognitive Abilities • Orientation o What is your name, today’s date, where we are, and the president’s name?  Patient is alert to person, place, time, and situation. Patient is awake, alert, and oriented X 3 • Memory o Immediate- Repeat this phrase for me: The sky is blue. o Recent- Name the four objects I showed you earlier (pen, measuring tape, watch, and reflex hammer) o Remote- What high school did you attend? o Patient’s immediate, recent, and remote memory are intact • Abstract Reasoning o What does it mean by “the early bird gets the worm?” o Patient’s abstract reasoning is intact. • Calculation o Starting at 50, can you subtract by 8 until you get to 2? o Patient’s calculation abilities intact. Cranial Nerves: name, number, test • CN 1 Olfactory o Test patency of nostrils.
