- Health of close family members especially those living at home
- Family HX of CAD, HTN, CVA, DM, CA, mental illness, blood dyscrasia
- Family tree (genogram)

ANSWER - Self esteem/self concept (You may have gotten some of this from
perception of health responses)
- Activity/exercise
- Sleep/rest
- Nutrition/elimination (physical ability)
