SMART wordt geformuleerd met het doel, specifiek, meetbaar, acceptabel,

Mevr A. is bekend met Nierfalen ook wel nierinsufficiëntie of nierschade genoemd.
Hiervoor wordt mevr twee keer in de week behandeld bij de dialyse in het CWZ.
Mevr is 71 jaar en woont in Nijmegen. Mevr heeft één zoon en is gehuwd. Mevr is
opgenomen in het herstelcentrum wegens een herseninfarct rechts. Mevr kon niet

Krachtsvermindering linkerarm hand en linkerbeen namen flink toe. Mevr werd thuis
gedragen en verzorgd door zoon. Mevr hoopt weer naar huis te kunnen na herstel.

Doel: Mevr kan zich zelfstandig wassen voor de wastafel
P: Niet zelfstandig kunnen wassen/aankleden/douchen

S: Onvoldoende evenwicht balans
SMART: Mevr kan over 2 weken door middel van een hulpmiddel haar bovenlichaam
zelfstandig wassen in de rolstoel voor de wastafel.

Doel: Mevr is zelfstandig in toiletgang.
P: Mevr heeft hulp nodig bij de toiletgang

S: Mobiliteit vermindering
SMART: Mevr kan over 1 week doormiddel van omstappen zelfstandig naar de toilet.

Doel: Mevr krijgt voldoende vocht en voeding binnen
