WGU D225 FIELD EXPERIENCE HOURS LOG 2024 3/5/ (4.5hrs) HOSPICE On this day, my preceptor and I cared for 2 patients with their ages between 70 and 95 yrs old. We cared for a male on Hospice for prostate cancer. This patient had been dx with prostate cancer with mets to his lungs and various other places. This patient had undergone treatment at first, but the cancer returned. The patient was living at home alone and as the cancer progressed, he became too weak to care for himself. The patient was placed in a long-term care facility and then decided to consult Hospice. I observed my preceptor ask the patient about pain management, supply needs, and psychosocial needs. The cancer was causing this patient to have generalized pain and my preceptor wanted to ensure his pain was well managed. The patient stated his pain was not being managed and he was still in severe pain. I observed my preceptor consult with the nurse taking care of this pt in the facility and the nurse stated that the patient never asks for his PRN pain medication. With education to both the patient and the nurse about the need to ask for pain meds when in pain and the need to ask the patient if they are having pain, a new plan was set in place to help better manage this pt’s pain.
