I. Inhibitoren van cyclo-oxygenase
Onderverdeling
• Analgetica – antipyretica
• Niet-steroidale anti-inflammatoire farmaca (NSAID’s)
1. Indicaties
• Koorts o.a. bij griepaal syndroom
o Eerste keuze: paracetamol
o Acetylsalicylzuur
o Sommige NSAID’s
• Pijn o.a. rugpijn, gewrichtspijnen, hoofdpijn, tandpijn, menstruatiepijnen,
postoperatieve pijn
• Acute of chronische inflammatie:
o Reumatoïde artritis
o Osteoartrose
o Spondylitis ankylosans
o Jicht (symptomatische therapie)
• Preventie van cardiovasculaire aandoeningen: acetylsalicylzuur in lage dosis (zie
geneesmiddelen ivm cardiovasculair stelsel)
2. Werkingsmechanisme
Twee types cyclo-oxygenase: COX-1 en COX-2
•
• COX-1: constitutief in
o Maag (gastroprotectie)
o Bloedplaatjes (aggregatie)
o Nier (onderhoud renale bloedflox)
• COX-2
o Geïnduceerd in inflammatoire cellen door cytokines zoals TNF en IL 1
o Toch ook constitutieve expressie in o.a. nier
o Gewenste effecten van NSAIDs berusten op inhibitie van COX-2
• NSAIDs
o Klassieke NSAIDs remmen zowel COX-1 (bijwerkingen!) als COX-2
(gewenste effecten) met geringe selectiviteit voor een van beide enzymen
o Ontwikkeling van COX-2 selectieve NSAIDs: meer dan 5-voudig COX-2-
selectief
• Inhibitie van COX
o Competitief reversibel, behalve voor acetylsalicylzuur
o Acetylsalicylzuur: acetylatie van serine in positie 530 (actieve zone van
enzym) = irreversibele inhibitie
• Paracetamol inhibeert selectief COX in centrale zenuwstelsel

, 3. Bijwerkingen
3.1. Gastro-intestinaal
• Erosies (dyspepsie, diarree, nausea), ulcera, bloedingen en perforaties
• Vooral thv maag, minder thv dunne darm, zelden thv dikke darm
• Verschil in frequentie naargelang het product
o Paracetamol: nagenoeg geen irritatie van gastro-intestinale tractus
o Piroxicam > diclofenac, naproxen, indometacine > ibuprofen (GI bloedingen)
o Minder uitgesproken bij COX-2-selectieve NSAIDs
• Mechanisme
o Lokaal etsend effect op mucosa (alleen voor acetylsalicylzuur)
o Inhibitie van gastrisch cyclo-oxygenase dat instaat voor synthese van gastro-
protectieve prostaglandines (inhibitie maagzuursecretien, cytoprotectie)
→ treedt op onafhankelijk van de toedieningsweg
→ verklaart associatiepreparaten van diclofenac + misoprostol en naproxen +
esomeprazol
3.2. Renaal
• Acute renale insufficiëntie
• Bij personen met geactiveerd noradrenerg of renine-angiotensine-systeem
(hartinsufficiëntie, levercirrose met ascites, hypovolemie)
→ renale vasoconstrictie
→ gecompenseerd door renale synthese van vasodilaterende prostaglandines
(PGE2, PGI2)
• Bij NSAIDs → verminderde synthese van compenserende prostaglandines → daling
van renale plasmaflow en glomerulaire filtratie
• Binnen uren tot dagen na gebruik
• Reversibel
