,Hoofdstuk 1: Farmacotherapie van inflammatie

1 Inleiding

1.1 Effecten van prostaglandines in gezond weefsel

CZS − PGE2: zorgt voor verhoging lichaamstemperatuur.

Luchtwegen − PGI2 en PGE2: bronchodilatatie.

− PGF2α en TXA2: bronchoconstrictie.

Hart in utero − PGI2 en PGE2: houden ductus Botalli open (= verbinding tussen aortaboog en truncus pulmonalis).




Bloedvaten − Onder normale omstandigheden: beiden zijn in evenwicht in een gezond weefsel.

➢ Endotheelcellen stellen prostacycline vrij (PGI2):

o PGI2 remt bloedplaatjesaggregatie.

o PGI2 relaxeert bloedvaten ⇒ vasodilatatie.

➢ Bloedplaatjes stellen thromboxaan vrij (TX):

o TX stimuleert bloedplaatjesaggregatie.

o TX contraheert bloedvaten ⇒ vasoconstrictie.

− In pathologische omstandigheden:

➢ Versnelde degeneratie van endotheel ⇒ minder PGI2-synthese ⇒ evenwicht schuift naar

vasoconstrictie en bloedplaatjesaggregatie.

Maag − PGE2:

➢ Vasodilatatie bloedvaten maag: betere perfusie van de maagmucosa.

➢ Meer mucusproductie in binnenkant van de maag: bescherming tegen maagzuur.

➢ Minder maagzuursecretie (HCl).



− Deze 3 zijn belangrijk voor behouden van integriteit van maagmucosa.

Nieren − PGI2 en PGE2: ↑ perfusie.

− PGF2α: ↓ perfusie.

Geslachtsorganen − Bij de vrouw:

➢ Baarmoedercontracties: initiëren van de arbeid.

➢ Baarmoederhals en baarmoedermond relaxeren: bij bevalling.

➢ Leiden tot menstruatiepijn.



− Bij de man:

➢ Prostaglandines spelen een rol in de fertiliteit.

,1.2 Prostaglandines en inflammatie

Arachidonzuur (AA) veresterd aan de glycerolgroep van lipidendubbelmembranen

Lipase: knipt esterverbinding.

Actiever bij weefselbeschadiging.

Vrij arachidonzuur



COX-1: continue expressie. COX-2: meer expressie in ontstekingscellen.



Prostaglandines (PG). Prostaglandines ↑ op plaats van weefselbeschadiging.




Lokale vasodilatatie: rubor + calor. Omgezet tot weefselspecifiek PG.

Verhoogde permeabiliteit: tumor. Bijvoorbeeld in endotheel van bloedvaten: PGI2.

Inwerking op pijnzenuwen: dolor.




PG-receptor is PG-specifiek.

Bijvoorbeeld in endotheel van bloedvaten: PI.

,2 Niet steroïdale anti-inflammatoire geneesmiddelen

2.1 Algemeen NSAID’s

Chemie – Heterogene groep van verbindingen (meeste zijn zwakke organische zuren).

– Volgens hun chemische basisstructuur onderscheidt men verschillende groepen NSAID’s.

– Aspirine = prototype NSAID.

Werking – Meesten inhiberen COX reversibel ⇒ remmen synthese prostanoïden (PGs en TXs).

– Enkel aspirine inhibeert COX-1 en COX-2 irreversibel.

➢ Inhibitie COX-1 ⇒ ongewenste effecten door interferentie met de normale homeostase.

➢ Inhibitie COX-2 ⇒ analgetische, antipyretische en anti-inflammatoire effecten.



– Inhibitie COX ⇒ verhoogde aanmaak leukotriënen door grotere aanbod substraat aan enzyme

lipoxygenase (LOX).

Farmacodynamiek Examenvraag: “Bespreek bijwerkingen NSAIDS adhv fysio effecten van prostaglandines.” (vorige jaren)

– Calor, rubor, tumor: anti-inflammatoir effect door ↓ vasodilatatie, oedeem en capillaire lekkage.

– Dolor: pijnstillend effect is krachtiger naarmate inflammatie en weefselschade aan basis liggen van pijn.

– Koorts: lichaamstemperatuur wordt geregeld door hypothalamus.

➢ Staat onder controle van PGE2 dat gevormd wordt door COX-3 onder invloed cytokines (bv. IL-1).

➢ PGE2 stimuleert hypothalamus tot verhogen van lichaamstemperatuur.

➢ Remmen van PGE2-synthese voorkomt dit stimulerend effect ⇒ koortswerend.



– Anti-aggregerend: aspirine zal COX-1 irreversibel inhiberen.

➢ Hierdoor zal aanvankelijk zowel PGI2 als TXA2 wegvallen.

➢ Aangezien endotheelcellen in staat zijn om nieuwe PGI2 aan te maken (door de aanwezigheid van

DNA en alle organellen nodig voor de productie) zal het evenwicht verstoord worden en kantelen

naar de inactivatie van bloedplaatjes. Hierdoor zal er minder bloedstolling optreden.

➢ Bloedplaatjes kunnen dit COX-1 niet opnieuw aanmaken (hebben geen kern noch organellen).

➢ Het duurt hierbij dus 5 dagen (= tijd nodig voor aanmaak nieuwe BPL) vooraleer er terug genoeg

COX-1 is en het evenwicht dus terug hersteld wordt.

, Farmacokinetiek – Absorptie: de zwakke organische zuren worden goed geabsorbeerd.

– Opname: orale biologische beschikbaarheid wordt weinig beïnvloed door voedsel. Het voedsel zal

opname wel vertragen, maar totale geabsorbeerde hoeveelheid zal hetzelfde zijn.

– Hoge proteïnebinding (>98%): interageren met andere medicatie die ook hoge proteïnebinding hebben.

– Excretie: vooral renale klaring.

– Eliminatie: op basis van t1/2 onderscheidt men 2 groepen:

➢ NSAIDS met korte halfwaardetijd (< 6h) ⇒ 3 tot 4 doseringen/dag.

➢ NSAIDS met lange halfwaardetijd (> 10h) ⇒ 1 tot 2 doseringen/dag.

Indicaties – Indicaties:

➢ Onderdrukken van koorts.

➢ Behandeling van menstruatiepijnen.

➢ Ductus arteriosus sluiting bij neonaten.

➢ Inflammatoire pijn: post-operatieve pijn, hoofdpijn, tandpijn, rugpijn, spierpijn.

➢ Bij weefselschade en inflammatie van locomotorisch stelsel.

o De symptomen nemen af en het werkt pijnstillend, maar het stopt niet de ziekteprogressie.

o Bijvoorbeeld: RA, arthrose, ankyloserende spondylitis…

Contra-indicaties Juist/fout: “Zwangere vrouw kun je behandelen met NSAID’s.” (2013)

en risicopopulaties Juist/fout: “Effecten NSAID’s bij zwangere vrouwen: bloeding bij moeder en kind, premature arbeid, effect

lactatie, sluiten ductus Botalli.” (2014)

– Contra-indicaties:

➢ Hartfalen en hypertensie: wegens interferentie met normale water- en zouthuishouding.

➢ Geschiedenis GI ulcera en/of bloedingen: preventie door maagprotectie (PPI).

➢ Ouderen (>65 jaar): preventie door maagprotectie (PPI).

➢ Zwangerschap:

o Vertraagde inzet en verloop van de arbeid (tocolyse).

o Langere bloedingstijd bij moeder en kind.

o Vroegtijdig sluiten van ductus Botalli bij neonatus ⇒ prenataal rechterhartoverbelasting.
