Basis gynaecologische ingrepen: 1. cervicaal kanaal gedilateerd tot 10 mm (nl toegankelijk voor instrument 4 mm)

Vaak gefractioneerde curettage: eerst endocervicaal kanaal, dan cavum corpus van uterus

Nu indien vermoeden van carcinoom; gefractioneerd curettage; eerst uit cervicaal kanaal dan uit urethrale caviteit

Endometriumbiopsie: Pipelle de Cornier in bm holte gebracht: vacuumtrekken door opturator
➢endometrium opgezogen/ biopsie genomen
Zo representatieve biopsies van over hele caviteit van bm
o fertiliteitskliniek: meting/datering van endometrium
o Bij verdikt endometrium en abnormaal bloedverlies na menopauze
o Zo weinig carcinoma missen en is ook geen te vervelende interventie (5-6/10 pijn)
, Hysteroscopie:
Ambulante hysteroscopie - cervix gedilateerd + scoop gebracht in uterus
- kleine afwijkingen zoals poliepen, of hyperplasie van het endometrium, of subendometriale myomen gevisualiseerd
- Cavum opgeblazen onder druk met vloeistof
- Diagnostische hysteroscopie aangevuld met operatieve hysteroscopie:
o Biopsies
o ingrepen via hysteroscoop
bv. poliepectomie, verwijderen intrauterien septum, volledige endometrium gereseceerd en/of gecoaguleerd
(hysteroscopische endometriumablatie)
Niet veel interventie mee kunnen doen
Bij vermoeden van carcinoma; !!! geen histeroscopie
• dun endometrium en geen staal bekomen dan ambulante histoscopie
Lichtbron voor goede verlichting van bm holte
Ingrepen: operatieve hysteroscopie Snijden en coaguleren
Vocht aan en afvoeren
Onder algemene narcose
Bij zware bloedingen; bvb door poliep etc dan zo behandelen !!!!!niet bij carcinoom anders verspreiding van carcinoma (dus bij
vermoeden niet gedaan)

- via de navel laparoscoop ingebracht

Ingrepen: Laparotomie = openen van abdomen via open incisie
Insnede:
- mediaan (navel naar pubis)
- horizontaal (Pfannenstiel – of “bikini insnede”) aan bovengrens pubisbeharing
Onder bikinilijn: onder grens van pubisbeharing; is horizontale insnede
Carcinoma operatie: nood aan wegnemen van lymfeklieren etc dan nood aan verticale incisie, soms tot navel
