Radiotherapie Blok 2College 1
- ICRU houdt zich bezig met onder andere de eenduidigheid van het definiëren
van te bestralen doelvolumes en doseringsmethoden. Doel om wereldwijd
uniformiteit te krijgen in doseringen doelgebied. ICRU is belangrijk bij de
treatment planning, planning van de bestraling. Arts geeft het doelgebied aan
met een cirkel.
- GTV = gross tumor volume, zichtbare
grootte van de tumor die te zien is op
de CT-scan
- CTV = clinical target volume, groter
gebied bestralen dan de tumor is >
voor uitbreidingen/uitzaaiingen van de
tumor die nog niet zichtbaar zijn.
- PTV = planning target volume, voor
(inwendige)beweging van de patiënt;
voor de onnauwkeurigheid. Ook voor
afwijkingen van het apparaat/tafel.
PTV altijd zo klein mogelijk.
- Deze waarden worden gebruikt om onnauwkeurigheden in
het proces van bestraling minder invloed te laten hebben.
- TV = treated volume, het gedeelte in de patiënt wat 95% of
meer straling krijgt (het rode gebied). Bij een palliatieve
behandeling wordt vaak gebruik gemaakt van twee bundels
(B), waardoor het treated volume hoger is dan bij een
curatieve behandeling, waar meerdere bundels worden
gebruikt (A).
- Irradiated volume (IR)= Het volume wat aangestraald
wordt; hoeveel straling er op het omliggende weefsel komt
(blauwe op de afbeelding).
- NR = normeringspunt; PDD punt 100%. Arts geeft aantal
cGy en het punt waar het moet komen; door bundels aan te
passen kun je het normeringspunt verplaatsen.

- Verschillende energieën (links 10MV, rechts 20MV)
- Hoe groter het veld, hoe meer kans op verstrooide straling in de PDD-curve.
- Wanneer de patiënt verder weg ligt van de bundel (grotere FHA), neemt de
absolute dosis af. Hoe verder de patiënt van het focus ligt, des te betere
PDD > meer loodrecht komende straling.
- Bij grotere FHA > PDD neemt toe (relatieve dosis)
- PDD op dwarsprofiel = relatieve dosisverdeling op een of meerdere diepten >
loodrecht op de centrale as (dwars).
