CHAPTERS 6,7,8 INTRO TO MATERNITY
AND PEDIATRIC NURSING QUESTIONS
AND ANSWERS WITH SOLUTIONS 2024
interventions for ruptured amniotic fluid sac - ANSWER -note time, color, and amount of fluid
-notify HCP
-amniofusion technique done: IV fluid is infused into the amniotic cavity through an intrauterine
pressure catheter, maybe performed to add cushion around the cord
-may be ruptured artificially (amnitotomy) by HCP
-normal color of fluid: flecks of white vernix (fetal skin protectant)
7 mechanisms of labor - ANSWER as the fetus descends into the pelvis, it undergoes positional changes
so it adapts optimally to changing pelvic shape and size
aka cardinal movements
1. descent
2. engagement
3. flexion
4. internal rotation
5. extension
6. external rotation
7. expulsion
1. descent - ANSWER -required for all other mechanisms of labor to occur and for infant to be born
-occurs as each mechanism of labor comes to play
-station: the level of the presenting part (usually the head) in the pelvis
~estimated in cm
~minus stations are above the ischial spines
~plus stations are below the ischial spines
~as fetus descends, minus #'s decrease & plus
, numbers increase
2. engagement - ANSWER -occurs when the presenting part (usually the bi-parietal diameter of the fetal
head) reaches the level of the ischial spines of the mothers pelvis (presenting part is at a 0 station or
lower)
-often occurs before the onset of labor in a nullipara mother
-may not occur until well after labor begins in a multipara mother
3. flexion - ANSWER -fetal head should be flexed to pass most easily through the pelvis
-contractions increase the amount of fetal head flexion until the fetal chin is on the chest
4. internal rotation - ANSWER -when the fetus enters the pelvis head first, the head is usually oriented so
the occiput is toward the mothers right or left side
5. extension - ANSWER -as fetal head passes under the mothers symphysis pubis, it must change from
flexion to extension so it can properly negotiate the curve
6. external rotation - ANSWER -when head is born in extension, the shoulders are crosswise in the pelvis
and the head is somewhat twisted in relation to the shoulders
-the head turns farther to the side as the shoulders rotate within the pelvis
7. expulsion - ANSWER -the anterior shoulder then the posterior are born, followed by the rest of the
body
leopold's maneuver - ANSWER -determination of the fetal position and presentation by abdominal
palpitations
-helpful in locating the fetal back (best location for hearing FHR)
amniotomy procedure - ANSWER -HCP ruptures the membrane artificially
-color, odor, and amount of fluid are recorded
